To determine the date of conception?
Hello Doctor, my last menstrual cycle was around February 24-26.
I had unprotected intercourse with my boyfriend on March 9 (with ejaculation inside).
Then, I had sexual intercourse with another man on March 19 (without ejaculation inside).
I took emergency contraception within 6 hours after that.
I took a home pregnancy test on March 29 and it showed positive.
On March 30, a vaginal ultrasound showed a gestational sac measuring approximately 0.51 mm.
Is it possible that the pregnancy is from the second man? I read online that a gestational sac cannot be detected within two weeks of conception.
Is this correct? Thank you, Doctor.

A transvaginal ultrasound can typically detect a pregnancy at around 5 weeks after the last menstrual period.
It appears that the pregnancy is likely not from partner A, but rather from the previous boyfriend.

More Info
Determining the exact date of conception can be quite complex, especially when multiple sexual encounters occur close together, as in your case. Based on the information provided, let's break down the timeline and analyze the possibilities.
Your last menstrual period (LMP) was around February 24-26, and you had unprotected intercourse with your boyfriend on March 9. Following that, you had another encounter with a different partner on March 19, where you used a withdrawal method (pull-out method) and took emergency contraception within six hours afterward. You then confirmed your pregnancy with a home test on March 29, and an ultrasound on March 30 showed an embryo measuring approximately 0.51 mm.
Understanding Conception Timing
1. Ovulation and Fertilization: Typically, ovulation occurs about 14 days after the first day of your last menstrual period in a regular cycle. If we assume a standard 28-day cycle, ovulation would likely have occurred around March 10. Sperm can survive in the female reproductive tract for up to five days, which means that intercourse on March 9 could potentially lead to conception if ovulation occurred shortly thereafter.
2. Emergency Contraception: Taking emergency contraception (EC) after the encounter on March 19 significantly reduces the likelihood of pregnancy from that encounter. EC is most effective when taken as soon as possible after unprotected intercourse, and it works primarily by delaying ovulation. Since you took the EC within six hours, it would have likely prevented ovulation from occurring if it was still pending.
3. Ultrasound Findings: The ultrasound on March 30 showing an embryo measuring 0.51 mm indicates that you were likely around 4-5 weeks pregnant at that time. This aligns with conception occurring shortly after your ovulation, which would suggest that the most likely date of conception was around March 9 or shortly thereafter.
Conclusion on Paternity
Given the timeline, it is plausible that the conception could be attributed to the encounter with your boyfriend on March 9. The timing of ovulation and the subsequent ultrasound findings support this possibility. The encounter with the other partner on March 19, especially after taking emergency contraception, would likely not have resulted in conception.
Clarification on Embryo Visibility
Regarding the claim that an embryo cannot be seen within two weeks of conception, this is somewhat misleading. While it is true that the embryo may not be visible immediately after fertilization, by the time of your ultrasound on March 30, it is reasonable to detect a gestational sac or an embryo if conception occurred around the time of ovulation.
Recommendations
If you have concerns about paternity or the implications of your encounters, consider discussing these with a healthcare provider or a counselor. Genetic testing can provide clarity if needed in the future. Additionally, if you have any further questions about your pregnancy or reproductive health, do not hesitate to reach out to a medical professional for personalized advice and support.
