Syphilis skin lesions on the glans?
Dear Chief Physician,
1.
I previously had secondary syphilis and completed a three-shot treatment at the end of March.
I noticed a significant reduction in some of the syphilitic lesions, but there are still some brown spots remaining.
I would like to ask if there are any oral medications or topical treatments that could help improve this condition, as it is quite bothersome.
2.
Recently, I have noticed white spots near the glans and urethra that resemble pimples.
I previously tried to squeeze one, and it released something that looked like a pimple.
I usually keep the area dry, and the spots haven't increased in size; they are sometimes visible and sometimes not.
Could this be a bacterial infection, or is it possibly due to taking hot showers?
Thank you.

Dr. Wu Yuxin reply Dermatology
The syphilis symptoms will take some time to resolve.
As for the lesions on the glans, they cannot be assessed without visual examination, so it is recommended to consult a dermatologist nearby for evaluation and treatment.

Addressing skin issues related to syphilis and concerns about penile health requires a comprehensive understanding of both the underlying conditions and the appropriate treatment options.
1. Post-Syphilis Skin Concerns: After undergoing treatment for syphilis, it is common for patients to notice residual skin changes, such as brown spots or discoloration. These changes can be attributed to the healing process of the skin after the infection has been treated. While the primary syphilis lesions may resolve, pigmentation changes can persist for some time.
For addressing these brown spots, topical treatments such as hydroquinone or retinoids may be beneficial, as they can help lighten hyperpigmented areas. However, it is crucial to consult with a dermatologist before starting any new medication or treatment to ensure it is appropriate for your skin type and condition. Additionally, maintaining a good skincare routine that includes sun protection is vital, as UV exposure can exacerbate pigmentation issues.
2. Concerns About White Bumps on the Penis: The appearance of white bumps on the glans (head) of the penis, particularly if they resemble pimples, can be concerning. These bumps could be a variety of things, including:
- Pearly Penile Papules: These are small, harmless bumps that are normal anatomical variations and do not require treatment.
- Folliculitis: This is an inflammation of the hair follicles, which can occur due to irritation or infection. If the bumps are itchy or painful, it may indicate an infection that could require topical antibiotics or antifungal treatments.
- Sebaceous Cysts: These are small, non-cancerous lumps that can occur when sebaceous glands become blocked. They are generally harmless but can be removed if they become bothersome.
It is essential to avoid squeezing or popping these bumps, as this can lead to further irritation or infection. Instead, maintaining proper hygiene and keeping the area dry is advisable. If the bumps persist, change in appearance, or are accompanied by other symptoms (such as pain, itching, or discharge), it is crucial to seek evaluation from a healthcare provider or dermatologist.
3. General Recommendations:
- Consultation: Given your history of syphilis and current skin concerns, it is advisable to consult a healthcare professional, preferably a dermatologist or urologist. They can provide a thorough examination and appropriate treatment options tailored to your specific needs.
- Avoid Irritants: Be cautious with the products you use in the genital area. Avoid harsh soaps or overly hot water, as these can irritate the skin and exacerbate any existing conditions.
- Hygiene Practices: Regularly clean the area with mild soap and water, and ensure it is thoroughly dried afterward. Wearing breathable, cotton underwear can also help reduce moisture buildup and irritation.
In conclusion, while some skin changes post-syphilis treatment are normal, persistent or concerning symptoms warrant professional evaluation. Proper skincare, hygiene practices, and timely medical advice are key to managing these issues effectively.
