Medication consultation
Hello Doctor, I have been experiencing symptoms of panic, anxiety, and obsessive thoughts for 20 years.
I received treatment 10 years ago, during which the doctor prescribed Lexapro along with exercise.
After about a year, I was able to work and socialize normally, and the doctor said I could stop the medication.
However, recently the obsessive thoughts have returned, and I went back for a consultation.
This time, I was prescribed Mirtazapine 10 mg to take before bedtime.
I have been taking it for a month and a half and currently feel very drowsy and a bit disoriented, often yawning.
I noticed that the medication list includes allergy medications, which are cold medications.
Can I not take cold medicine while on this medication? Is this drug effective for anxiety and obsessive-compulsive disorder, as it is primarily used to treat depression? Thank you, Doctor, for taking the time to respond.
Niming, 40~49 year old female. Ask Date: 2023/04/11
Dr. Lin Mianzhang reply Psychiatry
Hello: This medication does not conflict with cold medications.
Does the note on your prescription indicate that you have had an allergy to cold medications in the past? The first-line treatment for obsessive-compulsive disorder (OCD) is selective serotonin reuptake inhibitors (SSRIs), and the medication you previously took, Lexapro, belongs to this class.
The mechanism of action of Mindset also involves serotonin, so theoretically, it may be effective in treating OCD.
Currently, there are some case reports suggesting that Mindset can improve OCD symptoms, but more research is needed to confirm this.
If you have any concerns, it is advisable to discuss them with your physician.
Wishing you safety and health.

More Info
When it comes to managing anxiety disorders and obsessive-compulsive disorder (OCD), understanding medication options and their interactions is crucial for effective treatment. Your experience with anxiety and OCD over the past 20 years, including previous successful treatment with medications like Zoloft (sertraline) and lifestyle changes such as exercise, highlights the importance of a comprehensive approach to mental health.
Medication Overview
You mentioned that your current prescription includes Mirtazapine (known as "敏特思" in Chinese), which is primarily an antidepressant that can also help with anxiety. Mirtazapine is classified as a noradrenergic and specific serotonergic antidepressant (NaSSA). It works by increasing the levels of norepinephrine and serotonin in the brain, which can help alleviate symptoms of depression and anxiety. However, it is important to note that while Mirtazapine can be effective for anxiety, it is not typically the first-line treatment for OCD. Selective serotonin reuptake inhibitors (SSRIs) like sertraline or fluoxetine are generally preferred for OCD due to their specific efficacy in reducing obsessive thoughts and compulsive behaviors.
Side Effects and Interactions
You mentioned experiencing drowsiness and a sense of being "out of it," which can be common side effects of Mirtazapine, especially at the beginning of treatment or when the dosage is adjusted. The sedative effects of Mirtazapine can be beneficial for those who struggle with insomnia, but they can also lead to daytime drowsiness. If this side effect is bothersome, it is essential to discuss it with your prescribing physician, as they may consider adjusting the dosage or switching to a different medication.
Regarding your concern about taking cold medications while on Mirtazapine, it is crucial to be cautious. Some over-the-counter cold medications can contain ingredients that may interact with your antidepressant. For example, certain antihistamines can increase sedation when taken with Mirtazapine. Therefore, it is advisable to consult your healthcare provider or pharmacist before taking any new medications, including over-the-counter products, to ensure they are safe to use in conjunction with your current treatment.
Treatment Considerations
Given your history of anxiety and OCD, it is essential to adopt a holistic approach to your treatment. Here are some key considerations:
1. Medication Management: Regular follow-ups with your psychiatrist are vital to monitor the effectiveness of your current medication and make necessary adjustments. If Mirtazapine is not adequately addressing your anxiety and OCD symptoms, your doctor may consider reintroducing an SSRI or exploring other options.
2. Therapy: In addition to medication, cognitive-behavioral therapy (CBT), particularly exposure and response prevention (ERP), is highly effective for OCD. Engaging in therapy can provide you with coping strategies and tools to manage your symptoms more effectively.
3. Lifestyle Modifications: Incorporating regular physical activity, a balanced diet, and mindfulness practices can significantly enhance your overall well-being. Exercise is known to reduce anxiety and improve mood, while mindfulness techniques can help you manage intrusive thoughts associated with OCD.
4. Support Systems: Connecting with support groups, whether in-person or online, can provide a sense of community and understanding. Sharing experiences with others facing similar challenges can be incredibly validating and helpful.
5. Education: Understanding your condition and the medications you are taking can empower you to make informed decisions about your treatment. Don't hesitate to ask your healthcare provider questions about your medications, potential side effects, and alternative treatments.
Conclusion
In summary, managing anxiety and OCD requires a multifaceted approach that includes medication, therapy, lifestyle changes, and support systems. While Mirtazapine can be beneficial, it is essential to monitor its effects and consult with your healthcare provider regarding any concerns, especially regarding potential interactions with other medications. By taking an active role in your treatment and maintaining open communication with your healthcare team, you can work towards achieving better mental health and overall well-being.
