Chronic nasal congestion issues?

Hello Doctor, since I have been experiencing symptoms of autonomic nervous system dysfunction, I have had nasal congestion for the past seven years.
During this time, I have also encountered some psychosomatic symptoms (such as palpitations, gastrointestinal discomfort, and headaches), but the nasal congestion has been manageable.
However, after being diagnosed with COVID-19 earlier this year, I experienced a period of significant difficulty breathing, and I'm unsure if it was related to the virus.
Recently, my nasal congestion has become particularly severe, making breathing feel laborious, with one side of my nose always blocked.

I consulted an otolaryngologist, who mentioned the presence of enlarged nasal polyps, which could be obstructing airflow.
A pulmonologist indicated that I have chronic inflammation in my airways and poor lung function.
I am currently on medication and using a nasal spray for treatment.
I also run for half an hour every day.

What confuses me is that the psychiatrist suggested taking fish oil with extremely high purity EPA to see if it could improve my nasal congestion, while the pulmonologist said it might not be very effective.
Given that pure EPA fish oil is expensive, costing around 3,000 to 4,000 NT dollars a month, I would like to ask the doctor if it is necessary to take pure EPA fish oil.
After hearing the pulmonologist's opinion, I feel it may not effectively resolve my nasal congestion, so I would like to seek your advice.
Thank you!

Hello, EPA has anti-inflammatory and anti-allergic effects, which may help reduce nasal allergy reactions, or assist in maintaining a good mood and energy levels.
It does not directly alleviate nasal congestion.
You may consider using it for a period of time, such as three to six months, depending on your financial situation, and then evaluate based on your personal experience whether to continue its use.

More Info

Long-term nasal congestion can be a frustrating and debilitating condition, especially when it persists for several years, as you have experienced. The underlying causes of nasal congestion can vary widely, including chronic sinusitis, nasal polyps, allergies, or structural issues like a deviated septum. In your case, it seems that you have been diagnosed with nasal polyps, which can obstruct airflow and contribute to your symptoms. Additionally, the impact of your autonomic nervous system dysfunction and the recent COVID-19 infection may have exacerbated your respiratory issues.

Understanding the Causes
Nasal congestion can result from inflammation of the nasal passages, which may be due to chronic conditions such as allergic rhinitis or non-allergic rhinitis. In your situation, the presence of nasal polyps suggests that there is a chronic inflammatory process at play. These growths can block the nasal passages and lead to difficulty breathing through the nose. Furthermore, chronic inflammation in the respiratory tract can also affect lung function, as noted by your pulmonologist.

Treatment Options
1. Medical Management: Since you are already on medication and using nasal sprays, it's essential to ensure that these treatments are optimized. Intranasal corticosteroids are often the first line of treatment for nasal polyps and chronic rhinosinusitis. They help reduce inflammation and can shrink polyps over time. If you haven't seen significant improvement, discussing alternative medications or dosages with your ENT specialist may be beneficial.

2. Surgical Intervention: If medical management fails to alleviate your symptoms, surgical options such as endoscopic sinus surgery may be considered. This procedure can remove polyps and improve drainage in the sinuses, potentially leading to better airflow and reduced congestion.

3. Lifestyle Modifications: Regular aerobic exercise, such as your daily jogging, can improve overall respiratory function and may help alleviate some symptoms. Staying hydrated and using a humidifier can also help keep the nasal passages moist and reduce congestion.

Dietary Considerations
Regarding the suggestion from your psychiatrist to take high-purity EPA fish oil, there is some evidence that omega-3 fatty acids, particularly EPA, can have anti-inflammatory effects. However, the evidence regarding their effectiveness specifically for nasal congestion or polyps is limited. While some studies suggest that omega-3 supplementation may help reduce inflammation in general, it is not a guaranteed solution for nasal congestion.

Given the high cost of pure EPA fish oil and the uncertainty regarding its effectiveness for your specific condition, it may be wise to weigh the potential benefits against the financial burden. Consulting with a nutritionist or your healthcare provider about dietary sources of omega-3s, such as fatty fish (salmon, mackerel, sardines), walnuts, and flaxseeds, could be a more cost-effective approach to incorporate these beneficial fats into your diet.

Conclusion
In summary, managing long-term nasal congestion, especially with the presence of nasal polyps and chronic inflammation, requires a comprehensive approach that includes medical treatment, potential surgical options, and lifestyle modifications. While dietary supplements like EPA fish oil may offer some benefits, they should not replace conventional treatments. It is crucial to maintain open communication with your healthcare providers to tailor a treatment plan that addresses your specific needs and symptoms effectively. If you have further concerns about the effectiveness of your current treatment or the necessity of supplements, do not hesitate to seek a second opinion from a specialist.
