Hip and Leg Pain: When to Seek Medical Advice - Neurology

Medication Consultation

Hello Dr.
Peng: I often experience discomfort and aching in my right thigh near the hip joint, which extends to the thigh muscles.
Pressing on that area causes significant soreness.
When I sleep with my knees straightened or bent, I often hear cracking sounds, and I need to adjust my position forcefully to feel temporarily relieved, after which the cracking sounds do not return for a short time.
Additionally, the muscles on the right side of my calf feel tense and uncomfortable.
I'm unsure if this is related to a spinal or nerve issue.
The discomfort radiates from my buttocks down to the soles of my feet, primarily affecting the outer side of my right thigh and extending to the right side of my foot.
During these episodes, I often use topical ointments for relief.
Yesterday, a doctor at the clinic prescribed Mecobalamin capsules to supplement my vitamin B12 intake.
However, this morning, after taking one capsule on an empty stomach around 7:30 AM, I began to experience intermittent pain on the left side of my head shortly after, which has persisted into the evening.
The right side of my head feels completely normal.
The pain is concentrated above my left temple and extends to the left side of the back of my head.
I can feel a pulsing sensation when I lightly touch that area.
I would like to know if I should discontinue taking the capsules, as I experienced diarrhea once after taking them.
Should I rest for a few days without continuing the capsules, or should I visit a hospital and see a specialist?

Dr. Peng Shiwei reply Neurology

Hello, it is recommended that you take B12 after meals, which may help improve your symptoms.
As for the discomfort related to the hip, it seems to be more related to muscle or fascia issues.
It is advisable to consult a neurologist or a rehabilitation specialist.
Thank you for your inquiry.

More Info

It sounds like you are experiencing a complex set of symptoms involving your hip, leg, and even some neurological sensations in your head. Given the nature of your symptoms, it is important to approach this situation carefully and seek appropriate medical advice.

Understanding Your Symptoms
1. Hip and Leg Pain: The pain you describe in your right thigh, extending to the hip and down to the knee, could be related to several conditions, including hip joint issues, muscle strain, or nerve compression. The "clicking" sound you hear when bending your knee could indicate a mechanical issue within the joint, such as a labral tear or issues with the cartilage.
2. Radiating Pain: The discomfort that radiates from your hip down to your foot suggests that there may be nerve involvement, possibly due to conditions like sciatica or piriformis syndrome, where the piriformis muscle irritates the sciatic nerve.

3. Neurological Symptoms: The headache you experienced after taking the vitamin B12 supplement raises concerns. While B12 is generally safe, individual reactions can vary. The headache, particularly if it is unilateral (on one side), could indicate a migraine or tension-type headache, especially if you have a history of headaches. The fact that you experienced diarrhea after taking the supplement could also suggest an intolerance or sensitivity to the formulation.

When to Seek Medical Advice
Given the complexity of your symptoms, it is advisable to consult a healthcare professional. Here are some guidelines on when to seek medical attention:
- Persistent Pain: If your hip and leg pain persists or worsens despite conservative measures (like rest, over-the-counter pain relief, or topical treatments), it is essential to see a doctor.
- Neurological Symptoms: The onset of headaches, especially if they are new or different from your usual headaches, warrants medical evaluation. If you experience any additional neurological symptoms, such as weakness, numbness, or changes in vision, seek immediate medical attention.

- Gastrointestinal Reactions: If you continue to experience gastrointestinal symptoms like diarrhea after taking supplements, it may be wise to stop taking them and consult a healthcare provider for alternatives.

Recommended Actions
1. Consult a Specialist: Given your symptoms, it would be beneficial to see a specialist. An orthopedic doctor can evaluate your hip and leg pain, while a neurologist can assess your headache and any potential nerve-related issues.

2. Diagnostic Imaging: Depending on the findings from your initial consultation, your doctor may recommend imaging studies, such as X-rays or MRI, to assess the condition of your hip joint and surrounding structures.

3. Physical Therapy: If your pain is related to musculoskeletal issues, physical therapy may be recommended to strengthen the muscles around the hip and improve flexibility, which can alleviate some of the discomfort.

4. Medication Review: Discuss the vitamin B12 supplement with your doctor, especially if you suspect it may be causing your headaches. They can recommend appropriate dosages or alternative treatments.

5. Monitor Symptoms: Keep a log of your symptoms, including when they occur, their intensity, and any associated factors (like activity level or dietary changes). This information can be valuable for your healthcare provider in diagnosing and treating your condition.

Conclusion
In summary, your symptoms warrant a thorough evaluation by a healthcare professional. Given the potential for both musculoskeletal and neurological issues, a multidisciplinary approach may be necessary. Do not hesitate to seek medical attention, especially if your symptoms persist or worsen. Your health is paramount, and timely intervention can lead to better outcomes.
