The presence of semen flowing out while a catheter is in place?
Hello, I have been using a catheter for urinary retention for about 10 days now.
Since I still cannot urinate normally on my own, the doctor has decided to keep the catheter in for another week.
However, today I had a dream related to sexual matters and woke up to find that I had experienced a nocturnal emission.
After cleaning up, I performed routine antiseptic care with Betadine on the urethral opening.
What should I do next? Do I need to go to a urology clinic to change the catheter?

Dr. Li Minghui reply Urology
Hello: Nocturnal emissions themselves do not necessarily lead to catheter-related issues.
It is important to observe whether the urine is cloudy and if there is any discharge from the urethral opening, such as yellow discharge.
If both are normal and there is no fever, you can return for your follow-up appointment as scheduled.
Wishing you good health.

Experiencing semen leakage while having a catheter in place can be concerning, especially if you are already dealing with urinary retention and the complications that come with it. First and foremost, it is important to understand that the presence of a catheter can sometimes lead to unexpected physiological responses, including semen leakage, particularly if you have had a sexual dream or arousal while the catheter is in place.
In your case, since you have been using a catheter for about ten days due to urinary retention, it is essential to monitor your symptoms closely. Here are some steps you can take:
1. Maintain Hygiene: You mentioned that you have been cleaning the urethral opening with iodine solution. This is a good practice to prevent infections. Ensure that you continue to keep the area clean and dry. You may also want to use sterile gauze or a clean cloth to cover the area to absorb any leakage.
2. Monitor Symptoms: Keep an eye on any changes in your symptoms. If you notice increased pain, swelling, or any unusual discharge, it is crucial to seek medical attention promptly.
3. Consult Your Urologist: Given that you are still experiencing urinary retention and now have had an episode of semen leakage, it would be wise to consult your urologist. They can assess whether the catheter is still appropriately placed and functioning. They may also want to evaluate your urinary retention issue further, as prolonged catheter use can lead to complications such as urinary tract infections (UTIs) or bladder spasms.
4. Discuss Catheter Change: If you feel uncomfortable or if the catheter seems to be causing more issues, discuss the possibility of changing the catheter with your urologist. Sometimes, a different type of catheter or a change in size can alleviate discomfort and reduce complications.
5. Consider Psychological Factors: It’s worth noting that sexual arousal can occur even in the presence of a catheter, and this is a normal physiological response. If this becomes a frequent issue or causes you distress, discussing it with a healthcare professional may help you address any underlying concerns.
6. Follow-Up Care: Since your doctor has indicated that the catheter may need to remain in place for another week, ensure that you have a follow-up appointment scheduled. This will allow for a proper evaluation of your urinary function and any necessary adjustments to your treatment plan.
In summary, while semen leakage can be an unexpected occurrence with a catheter, maintaining hygiene, monitoring your symptoms, and consulting with your urologist are essential steps to take. They will provide the best guidance tailored to your specific situation and ensure that you receive the appropriate care moving forward. Remember, it’s always better to err on the side of caution when it comes to your health, especially with urinary issues and catheter use.
