CA19-9 Levels: When to Worry About Pancreatic Health - Gastroenterology and Hepatology

CA19-9 Issues

Hello doctor, three years ago my CA 19-9 level was over 5, and during a health check in February this year, it increased to over 15.
After another check two months later, it rose to over 17.
All other biochemical test results are normal, except for a fasting blood sugar of 101.
I would like to ask if this increase in CA 19-9 is concerning.
I have a history of anxiety and autonomic nervous system disorders, and I'm worried about potential pancreatic issues.
Thank you.

Mr. Long, 30~39 year old female. Ask Date: 2023/04/21

Dr. Chen Shidian reply Gastroenterology and Hepatology

CA 19-9 cannot be used to diagnose cancer.
If the abdominal ultrasound is normal, it can be monitored; however, if there are still concerns, a self-paid MRI may be considered.

CA19-9 is a tumor marker that is primarily associated with pancreatic cancer, but it is important to note that elevated levels can also be seen in a variety of other conditions, including benign diseases. The normal range for CA19-9 is typically less than 37 U/mL, and levels above this can indicate the need for further investigation, especially if there is a significant increase over time.

In your case, you mentioned that your CA19-9 levels have increased from 5 to 15 and then to 17 over a span of a few months. While these values are above the normal range, they are not extremely high, especially when compared to values that can reach several hundred or even thousands in cases of significant pancreatic pathology. It is also important to consider that CA19-9 is not specific to pancreatic cancer; it can be elevated in conditions such as pancreatitis, liver disease, bile duct obstruction, and even certain gastrointestinal cancers.

Given that your biochemical tests are normal and your fasting blood sugar is only slightly elevated at 101 mg/dL, it suggests that there may not be an immediate cause for concern regarding your pancreatic health. However, your anxiety regarding your health and the fluctuations in your CA19-9 levels are understandable, especially if you have a history of health anxiety and autonomic nervous system dysregulation.

When evaluating CA19-9 levels, it is crucial to correlate them with clinical symptoms and other diagnostic findings. If you are experiencing symptoms such as abdominal pain, weight loss, or changes in appetite, these should be discussed with your healthcare provider. In the absence of significant symptoms and with only a mild elevation in CA19-9, it may be reasonable to monitor the levels over time rather than jumping to conclusions about pancreatic cancer.

If you are particularly concerned, further imaging studies such as an abdominal ultrasound or a CT scan can be helpful in assessing the pancreas and surrounding structures. These imaging modalities can help identify any abnormalities such as masses, cysts, or signs of pancreatitis. Additionally, if there are any changes in your symptoms or if the CA19-9 levels continue to rise significantly, more aggressive diagnostic measures may be warranted.

In summary, while an increase in CA19-9 levels can be concerning, it is essential to interpret these results in the context of your overall health, symptoms, and other diagnostic findings. Regular follow-up with your healthcare provider, who can monitor your CA19-9 levels and any associated symptoms, is recommended. If you continue to experience anxiety about your health, consider discussing this with a mental health professional who can help you manage these feelings effectively.
