Chronic prostatitis and hematuria?

Hello Doctor, I was diagnosed with chronic prostatitis in 2018, and since then, I experience hematuria approximately once a year, usually occurring after exercise or a few days after hiking.
Each time I consult a doctor, they say everything is fine and that it might be due to microvascular rupture caused by exercise.
I have also undergone ultrasound and venography to confirm that there are no stones.
I would like to ask: 1.
Since I had never experienced hematuria before developing prostatitis, could the prostatitis be causing me to have hematuria more easily after exercise? 2.
If I experience hematuria after exercise, should I seek medical attention immediately, or can I observe for a few days, and if the hematuria does not continue, assume that everything is fine?

Hello:
1.
I have never experienced hematuria before my prostatitis.
Could the prostatitis be causing my exercise-induced hematuria? -- The two may not necessarily be related; it is also possible that the exercise itself is causing the hematuria.
2.
If I experience hematuria after exercising, should I seek medical attention immediately, or can I wait a few days to see if it resolves on its own? -- You should seek medical attention immediately; otherwise, if the hematuria disappears, it will be more difficult to determine the cause during examination.
The next time you experience hematuria, make sure to document in detail what happens over the 24 hours.
Additionally, make it a habit to drink plenty of water, and if you have high uric acid levels, you should manage them.
Wishing you good health.

More Info

Chronic prostatitis is a condition that can significantly impact a man's quality of life, and its relationship with hematuria (blood in urine) can be complex. Based on your history and the information provided, let's address your questions regarding the connection between chronic prostatitis and hematuria.

1. Could chronic prostatitis lead to exercise-induced hematuria?
While chronic prostatitis itself is not typically known to directly cause hematuria, it can contribute to conditions that may make the urinary tract more susceptible to bleeding. Chronic inflammation of the prostate can lead to increased vascularity and fragility of the blood vessels in the pelvic region. This could potentially make the blood vessels more prone to rupture during physical activities, especially if there is any jarring or impact involved, such as running or hiking. Additionally, if there is any associated pelvic floor dysfunction or increased pressure in the pelvic area due to the prostatitis, this could also contribute to the likelihood of experiencing hematuria during or after exercise.

2. Should you seek immediate medical attention for exercise-induced hematuria?
If you experience hematuria after exercise, it is important to monitor the situation closely. If the blood in your urine is minimal and resolves within a short period, it may not be a cause for immediate concern. However, if you notice significant amounts of blood, or if the hematuria persists, it is advisable to seek medical attention. Other concerning symptoms that would warrant immediate evaluation include pain during urination, back pain, fever, or any other unusual symptoms.
In your case, since you have a history of chronic prostatitis and have undergone various imaging studies that have ruled out stones or other structural abnormalities, it is reasonable to consider that your hematuria could be related to the physical stress of exercise in conjunction with your underlying condition. However, it is essential to maintain open communication with your healthcare provider about these occurrences, especially since they have been recurrent.

Additional Considerations:
- Follow-Up Care: Regular follow-ups with your urologist are crucial, especially since you have a history of chronic prostatitis. They may recommend periodic evaluations, including urinalysis and possibly imaging studies, to monitor for any changes in your condition.

- Lifestyle Modifications: Engaging in low-impact exercises may help reduce the risk of hematuria while still allowing you to maintain an active lifestyle. Additionally, staying well-hydrated can help dilute the urine and potentially reduce irritation in the urinary tract.

- Pelvic Floor Health: Consider discussing pelvic floor exercises with your healthcare provider. Strengthening the pelvic floor muscles may help alleviate some symptoms associated with chronic prostatitis and improve overall urinary function.

In conclusion, while chronic prostatitis may not directly cause hematuria, it can create conditions that make bleeding more likely during physical activity. Monitoring your symptoms and maintaining regular communication with your healthcare provider will be key in managing your condition effectively. If you experience any significant changes or new symptoms, do not hesitate to seek medical advice.
