Chronic prostatitis
Hello, doctor.
I have been suffering from chronic prostatitis and urethritis for a long time, along with multiple episodes of epididymitis for over ten years.
Now, whenever my immune system weakens, I end up with a fever and hospitalization, which has significantly disrupted my daily life and work.
Bacterial cultures show resistance to antibiotics, and the only effective medication left is a combination of antibiotics.
I also have diffuse calcifications and cysts in the prostate, as well as diverticula within the prostate.
I would like to know what surgical options are available for these conditions.
During discussions, it was mentioned that due to urethral stricture, the green light laser surgery would only be determined at the time of the procedure if it could be performed.
Typically, the endoscope can only accommodate sizes 10 to 14, or there is a risk of accessing through the perineum.
I am truly suffering and feeling very depressed; I really want to resolve this issue.
However, it seems that the only options I keep hearing about are the green light laser and radical treatment.
I would like to ask the doctor if there are really only these two methods available, or if there are other alternatives?

Chronic prostatitis is a complex and often frustrating condition that can significantly impact a person's quality of life. Your situation, characterized by long-term symptoms, recurrent urinary tract infections, and complications such as epididymitis, is not uncommon among those suffering from chronic prostatitis. The presence of antibiotic-resistant bacteria and the development of calcifications or diverticula in the prostate can complicate treatment options.
1. Understanding Chronic Prostatitis: Chronic prostatitis can be classified into several categories, with chronic pelvic pain syndrome (CPPS) being the most common. This condition can manifest as pelvic pain, urinary symptoms, and sexual dysfunction, and it often does not respond well to standard antibiotic therapy, especially when there is no bacterial infection present. The chronic nature of your symptoms suggests that you may be dealing with CPPS, which can be difficult to treat and may require a multifaceted approach.
2. Treatment Options: While you mentioned green light laser therapy and radical prostatectomy as potential options, there are other treatments that may be considered depending on the specifics of your case. Here are some options:
- Medications: In addition to antibiotics, other medications such as alpha-blockers (e.g., tamsulosin) can help relieve urinary symptoms by relaxing the muscles around the bladder neck and prostate. Pain management may also include nonsteroidal anti-inflammatory drugs (NSAIDs) or muscle relaxants.
- Physical Therapy: Pelvic floor physical therapy can be beneficial for patients with chronic pelvic pain. This therapy focuses on relieving tension in the pelvic muscles, which can contribute to pain and urinary symptoms.
- Interventional Procedures: If you have significant urinary obstruction due to prostate enlargement or strictures, procedures such as transurethral resection of the prostate (TURP) or laser therapy (including green light laser) may be appropriate. These procedures can help relieve urinary symptoms by removing obstructive tissue.
- Extracorporeal Shock Wave Therapy: This non-invasive treatment has shown promise in some studies for alleviating symptoms of chronic prostatitis by promoting healing and reducing inflammation.
- Prostate Massage: Some patients find relief through prostate massage, which can help to relieve congestion in the prostate and improve symptoms.
- Surgery: In cases where there are anatomical issues such as diverticula or significant calcifications, surgical intervention may be necessary. This could involve procedures to remove calcifications or to address any structural abnormalities in the prostate.
3. Psychological Support: Chronic prostatitis can lead to significant emotional distress, including anxiety and depression. It may be beneficial to seek support from a mental health professional who can help you cope with the psychological aspects of living with a chronic condition.
4. Lifestyle Modifications: Dietary changes, regular exercise, and stress management techniques can also play a role in managing symptoms. Avoiding irritants such as caffeine, alcohol, and spicy foods may help reduce urinary symptoms.
5. Consultation with Specialists: Given the complexity of your case, it may be beneficial to consult with a urologist who specializes in chronic prostatitis or pelvic pain syndromes. They can provide a comprehensive evaluation and tailor a treatment plan specific to your needs.
In conclusion, while green light laser therapy and radical prostatectomy are two options, there are multiple avenues to explore for managing chronic prostatitis. A multidisciplinary approach that includes medication, physical therapy, lifestyle changes, and possibly surgical intervention may provide the relief you are seeking. It is essential to have an open dialogue with your healthcare provider to discuss all available options and find the best course of action for your specific situation.
