Contraceptive pill issues
Hello, Doctor.
I have been taking Yaz for six months.
Today, while taking my medication, I accidentally lost one pill, so I have one less pill this month.
I am currently on the 23rd pill of this pack (which contains a total of 28 pills, with 4 being placebo).
My period is expected to start this Sunday, April 30th, and I was scheduled to start a new pack next Tuesday, May 2nd.
I would like to ask the doctor for advice on how to remedy this situation.
Should I open a new pack to replace the lost pill, or should I just continue with one less pill this month and start the new pack next Tuesday? Also, will missing one pill affect the timing of my period? Additionally, will missing one pill impact the contraceptive effectiveness? Thank you, Doctor.
An, 20~29 year old female. Ask Date: 2023/04/25

Taking one less pill should not have a significant impact, so you can continue to follow your original dosing regimen.

When you miss a birth control pill, it can understandably cause concern regarding both your menstrual cycle and the effectiveness of the contraceptive method. In your case, since you are using a combination oral contraceptive pill (OCP) like Yaz, here are some important points to consider:
1. Immediate Action: If you realize that you have missed a pill, the first step is to take the missed pill as soon as you remember. If you remember on the same day, take it immediately. If it’s the next day or later, take it as soon as you remember, but skip the missed pill if you are close to the time of your next dose.
2. Effect on Cycle: Missing one pill may not significantly affect your menstrual cycle, especially if you take the missed pill as soon as you remember. However, it can lead to some irregularities in your cycle, such as spotting or a delayed period. Since you are on the 23rd pill of a 28-day pack, you are nearing the end of your cycle, and your body may respond differently.
3. Starting a New Pack: You can either start a new pack immediately after finishing the current one or wait until the scheduled start date. If you choose to skip the missed pill and continue with the remaining pills, you should still be protected against pregnancy, provided you have taken the rest of the pills correctly.
4. Contraceptive Effectiveness: Missing one pill can reduce the effectiveness of the contraceptive, especially if it is missed during the first week of a new pack. Since you are already in the third week, the risk of pregnancy is lower, but it is still advisable to use a backup method of contraception, such as condoms, for the next seven days if you miss a pill.
5. Menstrual Timing: Missing a pill may cause your period to come earlier or later than expected. However, since you are nearing the end of your pack, your period should still arrive around the expected time, but be prepared for potential changes in flow or timing.
6. Consulting Your Doctor: If you are unsure or have further questions about your specific situation, it is always best to consult your healthcare provider. They can provide personalized advice based on your health history and current medications.
In summary, if you miss a birth control pill, take it as soon as you remember, continue with the rest of the pack, and consider using backup contraception for the next week. While one missed pill may not drastically affect your cycle, it’s essential to be aware of the potential for irregularities and reduced contraceptive effectiveness. Always consult with your healthcare provider for tailored advice and reassurance.
