Numbness in the face and hands?

Hello, doctor.
I have a slight tingling sensation around my right cheek and right ear.
Two years ago, it was around my ear, and now it’s on my cheek.
I can feel a soft, movable lump about one centimeter on my right cheek.
Visually, the nasolabial fold on the right side of my face is deeper and appears more sagging, with more excess fat on the right cheek.
My right eye has a very small double eyelid, while my left eye has a double eyelid.
There is no noticeable drooping of the eyelids, but my eyes are uneven in size; this has been the case since childhood, and it has become more pronounced in middle age.
I am unsure if these issues are related to the facial tingling.
My hands do not usually feel numb, but upon waking, both of my palms are very numb, which has been happening for about three to four years.
I have had medical examinations that showed slight narrowing at the C5-C6 vertebrae, and nerve tests showed no major issues, although there is a problem with carpal tunnel syndrome.
Could you please advise me on what might be causing these symptoms? Thank you.

Hello, Miss:
You mentioned several issues.
Clinically, we should first address the acute problems.
The "slight tingling sensation around the right cheek and right ear...
a soft, movable lump" may indicate potential compression of the trigeminal nerve, specifically the third branch, or a peripheral nerve.
The trigeminal nerve is responsible for transmitting sensory signals, and there are many causes of trigeminal neuralgia.
Approximately 80% of cases are due to compression by vascular structures as the trigeminal nerve exits the brainstem.
Other causes, such as vestibular schwannoma, meningioma, multiple sclerosis, and herpes zoster, can amplify normal sensory signals like touch and temperature into pain signals, leading to symptoms of trigeminal neuralgia.

I recommend checking your vitamin B levels and conducting a trigeminal nerve conduction study.
It would be advisable to consult a neurologist for further discussion and treatment.
Thank you for your inquiry, and feel free to discuss any related questions! Wishing you good health, peace, and happiness.
Dr.
Jiang Junyi, Taoyuan Medical Center, cares about you.

Facial numbness and tingling can be concerning symptoms, especially when they are accompanied by other physical changes or discomfort. Based on your description, there are several potential causes for the sensations you are experiencing in your right cheek and around your ear, as well as the changes in your facial appearance.

Firstly, the tingling and numbness in your right cheek could be related to nerve compression or irritation. Given your history of cervical spine issues, particularly the mild narrowing at the C5-C6 levels, it is possible that nerve roots originating from the cervical spine may be affected. This can lead to sensations of numbness or tingling in the face, especially if the nerves that innervate the facial region are involved. Cervical radiculopathy can sometimes manifest as facial symptoms, although it is more commonly associated with pain or numbness in the arms.

Additionally, the presence of a soft, movable lump on your right cheek raises the possibility of a benign growth, such as a lipoma or cyst. While these types of growths are generally not serious, they can sometimes exert pressure on nearby nerves, potentially leading to symptoms like tingling or numbness. It would be advisable to have this lump evaluated by a healthcare professional to determine its nature and whether it requires any intervention.

The asymmetry in your facial features, such as the deeper nasolabial fold and the difference in eyelid appearance, could also be contributing factors. Facial asymmetry can sometimes be associated with conditions affecting the facial nerve, such as Bell's palsy or other forms of facial nerve dysfunction. However, since you mentioned that these features have been present since childhood, they may simply be a part of your natural anatomy rather than a sign of an underlying neurological issue.

Your experience of hand numbness upon waking is another symptom worth considering. This could be related to positional factors during sleep, such as compression of the nerves in the wrists (carpal tunnel syndrome) or even in the neck. If you are experiencing numbness in your hands frequently, it may be beneficial to explore ergonomic adjustments to your sleeping position or consult with a specialist regarding potential nerve entrapment syndromes.

In summary, the combination of facial numbness, tingling, and changes in facial appearance could stem from several interconnected issues, including cervical spine problems, benign growths, and possibly nerve compression syndromes. It is crucial to consult with a neurologist or an ear, nose, and throat (ENT) specialist for a comprehensive evaluation. They may recommend imaging studies, such as an MRI of the cervical spine or facial structures, to better understand the underlying causes of your symptoms. Early diagnosis and appropriate management can help alleviate your concerns and improve your quality of life.
