Experiencing pain during ejaculation when the penis is not fully erect?
Hello, doctor.
Thank you for taking the time to answer my question: Sometimes after watching adult films for too long, when I switch to another video, my penis may be in a semi-flaccid state.
At that moment, it might feel a bit soft, but if I suddenly find an extremely exciting video that really appeals to me, the prolonged buildup can lead to excessive psychological arousal, while my penis hasn't fully caught up, resulting in ejaculation while still semi-erect.
I then experience a kind of cramping pain, as if the semen isn't being expelled smoothly.
However, in my daily life, whether urinating or masturbating, everything feels normal and painless; this pain only occurs in these specific situations.
If I just masturbate without watching adult films, I don't experience the aforementioned phenomenon.
This has happened a few times since I started watching adult films in middle school, and in my 20 years of life, this situation has occurred fewer than 10 times (I masturbate quite often), so the probability is very low.
I would like to ask the doctor if this situation is normal? (Note: Watching adult films for too long means over 2 hours).
Thank you again for answering my question, and I appreciate your effort!

Hello: If the occurrence is infrequent, there should be no problem.
Wishing you good health.

Experiencing painful ejaculation, especially in the context you've described, can be concerning but is not uncommon. Let's break down your situation and provide some insights into what might be happening.
Firstly, it's important to understand that sexual arousal and ejaculation are complex processes involving both physical and psychological components. When you watch adult films for an extended period, your body can become overstimulated. This overstimulation can lead to a state where your body is ready to ejaculate, but if your erection is not fully firm (as you described being in a "partial erection" state), the mechanics of ejaculation can become uncomfortable or even painful.
The pain you experience during ejaculation in this specific scenario may be due to a few factors:
1. Muscle Tension: During sexual arousal, various muscles in the pelvic region tighten in preparation for ejaculation. If you are in a state of heightened arousal but not fully erect, these muscles may contract more forcefully than usual, leading to discomfort or pain during ejaculation.
2. Prostate Sensitivity: The prostate gland plays a significant role in ejaculation. If the prostate is overly stimulated or if there is a buildup of pressure due to prolonged arousal without release, it can lead to discomfort during ejaculation.
3. Psychological Factors: The excitement and anticipation built up from watching adult films for an extended period can create a heightened state of arousal that may not align with your physical readiness. This mismatch can lead to a sensation of pain or discomfort when ejaculation occurs.
4. Dehydration or Friction: If you have been engaging in sexual activity for a long time without adequate lubrication, this can also lead to discomfort during ejaculation. Ensuring proper hydration and using lubrication can help mitigate this issue.
Given that you mentioned this has occurred less than ten times over the course of several years and only under specific circumstances, it may not be a cause for alarm. However, if the pain persists or worsens, or if you experience other symptoms such as blood in the semen, persistent pain, or changes in urination, it would be advisable to consult a healthcare professional.
In terms of normalcy, while occasional discomfort during ejaculation can happen, especially under the conditions you've described, it is not considered a standard experience. Most individuals do not experience pain during ejaculation, and if it becomes a recurring issue, it may warrant further investigation.
To summarize, while what you are experiencing can be attributed to a combination of physical and psychological factors related to arousal and ejaculation, it is essential to listen to your body. If the discomfort continues or if you have concerns about your sexual health, seeking advice from a urologist or a healthcare provider specializing in sexual health would be beneficial. They can provide a thorough evaluation and address any underlying issues that may be contributing to your experience.
