Missed Period: Should You Take a Pregnancy Test or Seek Medical Advice? - Obstetrics and Gynecology

Missed menstruation?

The last sexual intercourse was on December 17, 2022.
Following that, my menstrual cycles were approximately 25 days, with periods occurring in January to March, lasting about 6 days each and being dark red in color.
During this time, there was no sexual activity.
However, my period was expected on April 20 but has not arrived, and as of the early morning of April 26, there has been no sign of bleeding.
Should I take a pregnancy test? Could this be due to recent high stress and irregular sleep patterns? Or should I go for a check-up directly? I have previously experienced issues with hymenal healing.
Thank you, doctor, for taking the time to respond.
🙏

C, 10~19 year old female. Ask Date: 2023/04/30

When faced with a missed period, especially after a regular menstrual cycle, it is important to consider several factors before deciding whether to take a pregnancy test or seek medical advice. In your case, you mentioned that your last sexual encounter was on December 17, 2022, and that your menstrual cycle typically lasts about 25 days. Given that your period was expected on April 20 and has not arrived by April 26, it is reasonable to consider taking a pregnancy test.

Pregnancy tests are designed to detect the hormone human chorionic gonadotropin (hCG), which is produced shortly after a fertilized egg attaches to the uterine lining. If you are pregnant, this hormone can usually be detected in urine about one to two weeks after conception. Since your last sexual encounter was in December, and if you are indeed pregnant, you would likely have enough hCG in your system to yield a positive result by now. Therefore, taking a home pregnancy test could provide clarity regarding your situation.

However, it is also essential to consider other factors that may contribute to a missed period. Stress, changes in routine, and lifestyle factors such as sleep disturbances can significantly impact your menstrual cycle. High levels of stress can lead to hormonal imbalances, which may cause your period to be delayed or even skipped altogether. If you have been experiencing significant stress or changes in your daily routine, this could be a plausible explanation for your missed period.

If the pregnancy test is negative and your period still does not arrive, it would be advisable to consult with a healthcare provider. They can perform a more sensitive blood test to check for pregnancy and assess other potential causes for your missed period. Additionally, if you have a history of issues such as hymenal healing, it is important to discuss this with your doctor, as it may require further evaluation.

In summary, here are the steps you should consider:
1. Take a Pregnancy Test: Since you have missed your period, taking a home pregnancy test is a logical first step. Follow the instructions carefully for the most accurate results.

2. Monitor Symptoms: Pay attention to any other symptoms you may experience, such as breast tenderness, nausea, or fatigue, which could indicate pregnancy.

3. Consider Lifestyle Factors: Reflect on any recent changes in your life, such as increased stress, changes in sleep patterns, or significant lifestyle changes that could affect your menstrual cycle.

4. Seek Medical Advice: If the pregnancy test is negative and your period does not arrive, or if you have concerns about your reproductive health, make an appointment with a healthcare provider. They can provide a thorough evaluation and help determine the underlying cause of your missed period.

5. Discuss Any Concerns: If you have specific concerns about your hymenal healing or any other reproductive health issues, be sure to bring these up during your medical consultation.

Taking these steps can help you navigate your concerns about a missed period effectively and ensure that you receive the appropriate care and guidance.
