Missed Period After Trying to Conceive: Is Pregnancy Still Possible? - Obstetrics and Gynecology

If a pregnancy test is negative and menstruation has not occurred for over three weeks?

Hello, doctor.
I have been planning for pregnancy.
My period was due about three weeks ago, but I tested negative for pregnancy one week and two weeks ago.
This week, I visited a gynecologist who performed an abdominal ultrasound.
The doctor only noted that my period was likely approaching and mentioned that my uterus appeared normal.
However, I did not ask for detailed reasons at that time.
Now, three days later, my period still hasn't arrived, and I don't have any other significant symptoms.
I would like to inquire if there is still a possibility of pregnancy.
Additionally, the abdominal ultrasound should assess the thickness of the endometrium to determine if my period is imminent, but since the endometrium also thickens during pregnancy, could there be a possibility of misinterpretation? (P.S.
My last menstrual period lasted longer, and the doctor explained it was due to ovulation bleeding, so the total duration was about two weeks.)

A urine test can provide the necessary information, while an ultrasound can only be seen from the fifth week, making it not a very sensitive diagnostic tool in the early stages.

When it comes to missed periods and the possibility of pregnancy, there are several factors to consider, especially when you have been actively trying to conceive. Based on your description, it seems that you have taken multiple pregnancy tests that have returned negative results, and you have also undergone an abdominal ultrasound that indicated your uterus appears normal and that your menstrual cycle is likely approaching.

Firstly, it is important to understand that a missed period can occur for various reasons, not just pregnancy. Stress, hormonal imbalances, changes in weight, and other health issues can all contribute to irregular menstrual cycles. In your case, since you have been trying to conceive, it is natural to focus on the possibility of pregnancy. However, the negative pregnancy tests and the ultrasound findings suggest that pregnancy may not be the cause of your missed period.

Regarding the ultrasound, it is true that the thickness of the endometrial lining can provide insights into whether a menstrual period is imminent or if there is a possibility of pregnancy. In a typical early pregnancy, the endometrial lining does thicken to support the developing embryo. However, if the ultrasound did not show any signs of a gestational sac or other indicators of pregnancy, it is less likely that you are pregnant, especially given the negative urine tests.

You also mentioned a previous instance of what was described as ovulation bleeding, which can sometimes be mistaken for a light period. This can complicate the understanding of your cycle and ovulation timing. If your last menstrual period was unusually long or irregular, it could have affected your ovulation timing, leading to confusion about when you might have conceived.

In terms of the possibility of being pregnant despite negative tests, while it is rare, it is not impossible. Some women experience what is known as a "cryptic pregnancy," where they do not receive positive pregnancy tests until much later in the pregnancy. However, this is quite uncommon and usually involves specific circumstances. Given the information you provided, it seems more likely that your body is simply experiencing a delay in menstruation rather than an undetected pregnancy.

If your period does not arrive soon, it would be advisable to follow up with your healthcare provider. They may recommend further testing, such as a blood test for hCG (the hormone produced during pregnancy), which can detect pregnancy earlier than urine tests. Additionally, they may want to investigate other potential causes for your missed period, such as hormonal imbalances or other underlying health issues.

In conclusion, while there is a slight possibility of pregnancy, the combination of negative tests and ultrasound findings suggests that it is less likely. It is essential to monitor your symptoms and consult with your healthcare provider for further evaluation and guidance. They can provide you with the best advice tailored to your specific situation and help you understand your menstrual cycle better.
