Post-operative nursing issues for a patient after surgery?

Hello doctor, I underwent surgery for a hemorrhoidectomy and polypectomy on April 10.
After a month of recovery, I have noticed symptoms of anal fissure.
Currently, I experience bleeding after walking for a while, and when I soak in water and dry off at home, I also notice slight bleeding.
After soaking, I apply the medication "Zinc Oxide" prescribed by the doctor, but I feel that the discharge has increased, containing blood and some brownish sticky substance.
Is this normal? Should I also use the Neomycin/Proctosedyl ointment that I had previously?

Niming, 30~39 year old female. Ask Date: 2023/05/02

Dr. Xu Kaixi reply Surgery

It has been about a month since the fistula and polyp surgery on April 10th.
Theoretically, the wound should have healed by now.
If there are still secretions and symptoms, it indicates that the wound has not yet healed or there may be an infection.
It is recommended to return to the original surgical hospital for a follow-up examination.

Post-surgery care for hemorrhoids and anal fissures is crucial for recovery and to prevent complications. After undergoing surgery for hemorrhoids and polyps, it is not uncommon to experience symptoms such as bleeding or discomfort, especially if you are now noticing signs of an anal fissure. Here’s a comprehensive overview of what you should consider in your post-operative care.

Understanding Your Symptoms
1. Bleeding: Light bleeding after surgery can be normal, especially if you are experiencing anal fissures. However, if the bleeding is persistent or increasing, it is essential to consult your healthcare provider. The presence of blood mixed with mucus or a coffee-colored discharge may indicate that there is irritation or inflammation in the anal area.

2. Discharge: Increased discharge, especially if it is mixed with blood, can be a sign of irritation or infection. The use of topical medications like "紫箘素" (likely a topical treatment for fissures) can sometimes lead to increased moisture in the area, which may contribute to the sensation of more discharge.
3. Anal Fissures: These are small tears in the lining of the anus, often caused by passing hard stools or straining. They can be painful and may bleed during bowel movements. It’s essential to manage this condition effectively to promote healing.

Recommended Care Strategies
1. Dietary Adjustments: To prevent constipation and promote softer stools, increase your fiber intake. Foods rich in fiber include fruits, vegetables, whole grains, and legumes. Additionally, staying well-hydrated by drinking plenty of water is crucial.

2. Sitz Baths: Continue with sitz baths, as they can help soothe the area and promote healing. Soaking in warm water for about 15-20 minutes several times a day, especially after bowel movements, can provide relief.

3. Topical Treatments: You mentioned using "紫箘素." It is essential to follow your doctor's instructions regarding the application of this medication. If you have previously used "新黴素/樂癒痔" (likely a topical antibiotic or anti-inflammatory ointment), consult your doctor before combining treatments. Using multiple topical agents can sometimes lead to irritation or adverse reactions.

4. Avoid Straining: Straining during bowel movements can exacerbate fissures. If you feel the urge to go, try to respond promptly. If you are having difficulty, consider using a stool softener or a mild laxative, but consult your doctor first.

5. Pain Management: Over-the-counter pain relievers like acetaminophen or ibuprofen can help manage discomfort. However, always consult your healthcare provider before taking any new medications.

When to Seek Medical Attention
- Increased Bleeding: If you notice significant bleeding or if the bleeding does not stop after a few minutes of applying pressure, seek medical attention immediately.

- Severe Pain: If the pain becomes unbearable or does not improve with over-the-counter medications, consult your doctor.

- Signs of Infection: Symptoms such as fever, increased redness, swelling, or discharge that has a foul odor may indicate an infection and require prompt evaluation.

Follow-Up Care
Regular follow-up appointments with your healthcare provider are essential to monitor your recovery. They may recommend additional treatments or interventions if your symptoms do not improve.
In summary, while some bleeding and discharge can be normal after surgery for hemorrhoids and polyps, it is vital to monitor your symptoms closely. Implementing dietary changes, maintaining proper hygiene, and using prescribed topical treatments can significantly aid in your recovery. Always keep an open line of communication with your healthcare provider regarding your symptoms and treatment plan.
