Annoying acne?

Dear Doctor,
For the past few years, I have been troubled by acne.
I don't stay up late, eat light meals, maintain good hygiene, and drink plenty of water, yet my face remains oily, my pores are enlarged, and I have excessive body hair.
The acne I experience is mostly inflamed and red.
I've heard that popping pimples is not advisable, so I haven't done that, but it has left me with red spots.
Once, I mustered the courage to get a facial treatment, which cost me NT$1200, but the inflamed acne continued to appear, and I couldn't worry too much about it.
Although I did pop some pimples, they became very red but did not leave scars.
However, as a male, I feel embarrassed to visit a beauty salon.
The dermatologist prescribed medication for me, but it hasn't worked, and topical treatments have also been ineffective.

Recently, I came across an article online that resonated with my struggles, suggesting that I might have elevated androgen levels leading to oily skin and inflammation.
It recommended taking Androcur.
May I ask, Doctor, can I obtain this medication at a pharmacy? I have also researched that this medication may cause certain adverse effects, but I would rather risk erectile dysfunction than continue suffering from persistent acne, which has already impacted my personal development.
I can empathize with the patient who committed suicide due to acne (I won’t do anything stupid, but I really feel for that poor guy.
Give me death if life is so far from what one considers normal).
I live in central Taiwan; could you recommend a doctor who genuinely cares about patients' feelings? Is there a dermatology clinic that has a department for acne extraction? Please, I beg you, Doctor.

Dear William,
Don't be so pessimistic; I have never seen any acne patient that cannot be cured as long as they follow the dermatologist's recommendations.
First of all, you should have a thorough understanding of acne, including its causes, types, progression, and treatment options.
Avoid using Androcur due to its side effects; the best way to treat your acne may be oral retinoids.
You can visit Dr.
Bai Jia-Sheng's dermatology clinic in Taichung City; he is my junior and a very nice person.
Alternatively, you can see Dr.
Lin Bing-Huang, one of the best dermatologists for acne in Taiwan, who practices in Tainan City.
You can easily ask anyone in Tainan for directions to his clinic.

God created each person uniquely, and He loves you.
I hope this Christmas brings you hope and joy.
Wishing you peace, and may God bless you.

Dealing with persistent acne can be incredibly frustrating, especially when you feel like you've tried everything without success. It sounds like you've been proactive in managing your skin health by maintaining a good diet, staying hydrated, and practicing proper skincare. However, despite your efforts, you're still experiencing issues with oily skin, enlarged pores, and inflammatory acne. Let's break down some potential solutions and considerations for your situation.

First, it's important to understand that acne can be influenced by a variety of factors, including hormonal imbalances, genetics, diet, and skincare routines. Given your mention of high androgen levels, it’s possible that hormonal acne could be a significant contributor to your breakouts. Androgens, which are male hormones that can increase oil production in the skin, may lead to the type of acne you're experiencing.
You mentioned hearing about Androcur (cyproterone acetate), which is a medication that can help manage androgen levels and is sometimes used to treat hormonal acne. However, it's crucial to approach this option with caution. Androcur is a prescription medication and should only be taken under the supervision of a healthcare professional. Self-medicating, especially with hormonal treatments, can lead to unwanted side effects, including the potential for sexual dysfunction, mood changes, and other health issues. Therefore, I strongly advise against purchasing this medication without a doctor's guidance.

Instead, I recommend scheduling an appointment with a dermatologist who can assess your skin condition more thoroughly. A dermatologist can provide a tailored treatment plan that may include topical treatments, oral medications, or even hormonal therapies if deemed appropriate. They can also help you understand the underlying causes of your acne and suggest lifestyle changes that may further improve your skin.

In addition to medication, there are several non-pharmaceutical treatments that can be effective for acne management:
1. Topical Retinoids: These are derived from vitamin A and can help unclog pores and reduce inflammation. They are often recommended for treating acne and can improve skin texture over time.

2. Benzoyl Peroxide: This topical treatment helps kill acne-causing bacteria and can reduce inflammation. It’s available over-the-counter in various strengths.

3. Salicylic Acid: This is another common ingredient in acne treatments that helps exfoliate the skin and keep pores clear.

4. Chemical Peels: These can be performed by a dermatologist and can help reduce acne and improve skin texture.

5. Laser and Light Therapies: These treatments can target acne-causing bacteria and reduce inflammation.

6. Dietary Considerations: While you mentioned eating a light diet, some studies suggest that high-glycemic foods and dairy may exacerbate acne in some individuals. Keeping a food diary to track any correlations between your diet and breakouts may be helpful.

7. Stress Management: Stress can also play a role in acne flare-ups. Techniques such as mindfulness, meditation, or yoga may help reduce stress levels.

Regarding your concern about finding a compassionate dermatologist, I recommend looking for clinics that specialize in acne treatment or have a good reputation for patient care. Online reviews and recommendations from friends or family can be valuable resources. Many dermatology practices also offer aesthetic services, including acne extraction, which may be beneficial for your situation.

Lastly, it's essential to address the emotional toll that acne can take on your self-esteem and mental health. If you're feeling overwhelmed, consider speaking with a mental health professional who can provide support and coping strategies.

In summary, while it’s understandable to seek immediate solutions for your acne, it’s crucial to approach treatment thoughtfully and under professional guidance. A dermatologist can help you navigate your options and find a treatment plan that works for you, ultimately leading to clearer skin and improved confidence.
