Perianal mass
Hello Dr.
Chang,
Recently, I noticed a small soft and elastic lump at the anal opening while taking a shower.
It is asymptomatic, does not hurt when pressed, and is located only on one side of the anus.
There has been no bleeding with bowel movements, and I have a bowel movement once a day, but I'm not sure how long it has been there.
Here is a link to the image: https://imgur.com/a/IJnj7oz.
I would like to ask if this could be hemorrhoids? Should I see a colorectal surgeon for evaluation? Thank you.

Dr. Zhang Shuhao reply Surgery
The image is not very clear, but it is likely hemorrhoids.
You can start by taking warm sitz baths and maintaining smooth bowel movements.
If the symptoms become bothersome, please visit a colorectal specialist.

More Info
When it comes to soft anal masses, the most common concern is whether they are hemorrhoids or something else. Based on your description of a soft, elastic mass near the anal area that is painless and does not bleed, it is possible that you are dealing with a hemorrhoid, but there are other possibilities to consider as well.
Understanding Hemorrhoids:
Hemorrhoids are swollen veins in the lower rectum or anus, similar to varicose veins. They can be classified into two types: internal and external. Internal hemorrhoids are located inside the rectum and usually do not cause pain, while external hemorrhoids are located under the skin around the anus and can cause discomfort, especially if a blood clot forms. The fact that your mass is soft, elastic, and painless suggests it could be an external hemorrhoid, especially if it has been present for a while without causing significant symptoms.
Other Possibilities:
However, not all anal masses are hemorrhoids. Other conditions that could present as soft anal masses include:
1. Skin Tags: These are benign growths of skin that can occur around the anal area, often as a result of previous irritation or friction. They are typically painless and soft.
2. Anal Warts: Caused by the human papillomavirus (HPV), these can appear as soft, fleshy growths around the anus. They may be painless but can sometimes cause itching or discomfort.
3. Fistulas or Abscesses: An anal fistula is a small tunnel that forms between the end of the bowel and the skin near the anus, often resulting from an infection. An abscess is a collection of pus that can also occur in the anal area. These conditions may present with pain or discomfort, but they can sometimes be asymptomatic.
4. Other Benign Tumors: There are other benign tumors that can occur in the anal region, such as lipomas (fatty tumors) or other soft tissue tumors.
When to Seek Medical Attention:
Given that you have noticed this mass and it has been present for an unspecified duration, it is advisable to consult a healthcare professional, particularly a colorectal surgeon or a gastroenterologist. They can perform a physical examination and possibly other diagnostic tests to determine the exact nature of the mass.
While your mass may not currently be causing any symptoms, it is important to have it evaluated to rule out any serious conditions, including anal cancer, especially if there are changes in size, color, or if you start experiencing symptoms like bleeding, pain, or changes in bowel habits.
Conclusion:
In summary, while your description suggests that the mass could be a hemorrhoid or a benign skin tag, it is essential to have a healthcare professional evaluate it for an accurate diagnosis. Early assessment can provide peace of mind and ensure that if treatment is necessary, it can be initiated promptly. Don't hesitate to reach out to a specialist for a thorough evaluation.
