Chest tightness and difficulty breathing?
Hello, Doctor.
I would like to inquire about an incident that occurred in March this year.
While exercising, I suddenly felt unable to breathe, as if my airways were blocked.
After resting for a while, my condition improved.
I went to a health center where an electrocardiogram showed some oxygen deficiency and a slightly elevated heart rate.
Subsequently, due to issues with chest tightness, difficulty breathing, and tachycardia, I visited a hospital's cardiology and pulmonology departments for further examinations.
The electrocardiogram again indicated oxygen deficiency, but various blood tests returned normal values, and the echocardiogram was also normal.
Ultimately, the doctor performed a cardiac catheterization, which revealed no cardiovascular issues.
Three months later, I underwent a stress electrocardiogram, which was normal, but I still experience intermittent chest tightness, chest pain, difficulty breathing, and shoulder and neck pain.
Occasionally, I also experience palpitations at night while sleeping.
I have undergone these examinations at three major hospitals, and both cardiologists and pulmonologists have stated that there are no problems, yet I continue to have these symptoms.

Hello: Based on the symptoms you described, multiple hospital examinations have shown no cardiovascular or pulmonary issues.
The purpose of these tests was to rule out any life-threatening conditions or significant organ abnormalities.
Other less dangerous conditions that may cause these symptoms include: 1.
Psychological factors: Anxiety and stress can lead to palpitations, chest tightness, and respiratory discomfort.
Sometimes psychological stress can mimic symptoms of heart problems.
2.
Musculoskeletal issues: Neck and shoulder pain may be caused by muscle tension, postural problems, or cervical spine issues.
These problems can sometimes lead to chest discomfort and difficulty breathing.
3.
Gastroesophageal issues: Gastroesophageal reflux disease (GERD) or esophageal spasms may cause chest discomfort, breathing difficulties, and palpitations.
These issues are related to the digestive system but can sometimes present symptoms similar to heart problems.
You may try adjusting your dietary habits and avoiding triggers.
It is recommended to maintain a positive mood, avoid anxiety, keep a regular exercise routine, and have a normal daily schedule.
If you use tobacco or alcohol, try to avoid them as much as possible.
If symptoms are frequent, sometimes doctors may prescribe medications to help alleviate symptoms, so please follow up for further consultation.

More Info
Understanding chest tightness and breathing issues can be complex, especially when multiple medical evaluations return normal results. Based on your description, it seems you have undergone a thorough investigation, including electrocardiograms (ECGs), echocardiograms, and even cardiac catheterization, all of which have indicated no significant cardiovascular issues. This is reassuring, but it does not fully explain your ongoing symptoms of chest tightness, shortness of breath, and palpitations.
Potential Causes of Your Symptoms
1. Anxiety and Panic Disorders: Given that your symptoms include episodes of chest tightness and difficulty breathing, particularly during physical exertion or stress, anxiety could be a significant factor. Panic attacks can mimic cardiac symptoms, leading to feelings of breathlessness, rapid heart rate, and chest discomfort. If you find that your symptoms worsen during stressful situations or if you have a history of anxiety, this could be a contributing factor.
2. Musculoskeletal Issues: The mention of shoulder and neck pain suggests that musculoskeletal problems could also be at play. Tension in the neck and shoulder muscles can lead to referred pain in the chest and contribute to a sensation of tightness or discomfort. Poor posture, especially from prolonged sitting or computer work, can exacerbate these issues.
3. Respiratory Conditions: Although your lung function tests may have returned normal, conditions such as exercise-induced bronchoconstriction or asthma can cause symptoms similar to what you describe. These conditions might not always be evident in standard pulmonary function tests but can lead to episodes of breathlessness during exertion.
4. Cardiac Conditions: While your extensive cardiac evaluations have shown no significant issues, it is essential to consider that some conditions, such as microvascular angina or other forms of cardiac syndrome X, may not be easily detected through standard testing. These conditions can cause chest pain and discomfort without the presence of significant coronary artery disease.
5. Gastroesophageal Reflux Disease (GERD): GERD can cause chest pain and a sensation of tightness, especially after eating or when lying down. If you notice that your symptoms correlate with meals or specific positions, this could be worth exploring further.
Recommendations
1. Follow-Up with Specialists: Since you have already seen cardiologists and pulmonologists, consider consulting with a psychologist or psychiatrist, especially if anxiety or panic attacks are suspected. Cognitive-behavioral therapy (CBT) can be particularly effective in managing anxiety-related symptoms.
2. Physical Therapy: If musculoskeletal issues are suspected, physical therapy may help address any underlying muscle tension or postural problems. A physical therapist can provide exercises to strengthen and stretch the relevant muscle groups.
3. Lifestyle Modifications: Incorporate stress-reducing activities such as yoga, meditation, or regular exercise into your routine. These can help alleviate anxiety and improve overall physical health. Additionally, ensure you maintain a healthy diet and stay hydrated.
4. Monitor Symptoms: Keep a detailed diary of your symptoms, noting when they occur, their duration, and any potential triggers. This information can be invaluable for your healthcare providers in determining the underlying cause.
5. Further Testing: If symptoms persist despite these interventions, consider discussing with your doctor the possibility of further testing, such as a 24-hour Holter monitor or a stress echocardiogram, to evaluate your heart's response to physical activity more closely.
Conclusion
While your extensive testing has ruled out significant cardiovascular issues, the persistence of your symptoms suggests that other factors may be at play. A multidisciplinary approach involving mental health support, physical therapy, and lifestyle changes may provide relief. Always communicate openly with your healthcare providers about your symptoms and concerns to ensure comprehensive care.
