Why is only the upper eyelid of the right eye swollen when both eyes are experiencing allergies?
Dear Doctor,
My son is 20 years old and has had a history of eye allergies since childhood, although his symptoms are not as severe as they used to be.
Currently, he experiences symptoms of eye allergies, including tearing 1-2 times a day, itching in both eyes 2-3 times, and a swelling in the upper eyelid of the right eye (near the bridge of the nose) that is about 1 cm in size.
The swelling is not very red and has some flaking.
Although applying a topical treatment reduces the swelling for 2-3 days, it often recurs.
Notably, only the upper eyelid of the right eye experiences this issue, while the left eyelid has never been swollen (both eyes itch simultaneously, but for some reason, the left eyelid has never swollen).
Since childhood, my son has frequently had discharge from the right eye, and he has undergone invasive nasolacrimal duct examinations, which showed no issues with duct narrowing.
Doctors have attributed his tearing and itching to allergies, and I understand that it is challenging to identify allergens.
Additionally, this condition does not significantly disrupt his daily life, so I would like to focus on the swelling of the upper eyelid of the right eye (as it affects his appearance).
I would like to ask the doctor:
1) Since both eyes undergo the same scratching and rubbing actions due to allergies, why does only the upper eyelid of the right eye swell? Could there be a structural issue with the right eyelid?
2) If there is no structural issue with the right eyelid, is the information provided sufficient for the doctor to make a preliminary assessment of what the swelling in the upper eyelid might be? (Could it be eyelid eczema?)
Thank you, Doctor.

Hello: 1.
Eye allergies can lead to inflammation and swelling of the ocular tissues.
While your son is experiencing itching and rubbing of both eyes, it is puzzling why only the upper eyelid of the right eye is swollen.
It is possible that the tissues around the right eye are more sensitive to the allergen, or there may be other factors contributing to the swelling in that area.
Further evaluation is necessary to determine the specific cause.
2.
Based on your description, the swelling could be one possibility of eyelid eczema, but a diagnosis cannot be made solely based on symptom description.
Eyelid eczema is a type of dermatitis affecting the skin around the eyes, which may be related to allergic reactions, skin sensitivity, or other skin conditions.
However, other diseases or lesions could also cause similar symptoms, such as ocular infections, conjunctival cysts, seborrheic dermatitis, or chalazia.
Only an examination and assessment by an ophthalmologist can determine the underlying cause and provide appropriate treatment recommendations.

Swelling of the upper eyelid, particularly when it occurs only on one side, can be attributed to several factors, especially in the context of a history of allergies. In your son's case, the recurrent swelling of the right upper eyelid, accompanied by itching and tearing, suggests that it could be related to allergic reactions, localized inflammation, or even structural issues.
1. Allergic Reaction: Given your son's history of eye allergies, the swelling could be a localized allergic response. Allergies can cause the release of histamines, leading to inflammation and swelling in the affected area. The fact that he experiences itching and tearing in both eyes but only has swelling in the right eyelid could indicate that the right eyelid is more sensitive or reactive to allergens. This could be due to a variety of reasons, such as increased exposure to allergens on that side (like dust, pollen, or pet dander) or even a slight anatomical difference that makes the right eye more susceptible.
2. Structural Issues: While it is less common, structural issues could also contribute to unilateral eyelid swelling. For instance, if there is a slight anatomical difference in the eyelid or surrounding tissues, it could predispose that side to swelling. Additionally, if there is a history of previous infections or inflammation, scarring or changes in the tissue could lead to recurrent swelling.
3. Other Causes: Other potential causes of unilateral eyelid swelling include:
- Chalazion or Stye: These are localized swellings that occur due to blocked oil glands or infection of the eyelash follicles. They can cause swelling and discomfort, and while they typically affect the eyelid margin, they can also cause localized swelling.
- Eczema or Dermatitis: Contact dermatitis or eczema can cause localized swelling and irritation. If your son has a history of skin allergies, this could be a possibility, especially if the skin appears dry or flaky.
- Infection: Although less likely given the chronic nature of the symptoms, an infection such as conjunctivitis or a localized skin infection could cause swelling.
4. Diagnosis and Management: To address the swelling effectively, it is crucial to consult with an ophthalmologist or dermatologist. They can perform a thorough examination to determine the underlying cause of the swelling. Diagnostic tests may include:
- Slit Lamp Examination: This allows the doctor to examine the eyelid and surrounding structures in detail.
- Allergy Testing: If allergies are suspected, testing can help identify specific triggers.
- Imaging Studies: In rare cases, imaging may be necessary to rule out structural abnormalities.
In terms of management, if the swelling is indeed due to allergies, antihistamines or topical corticosteroids may be prescribed to reduce inflammation and itching. If a chalazion or stye is diagnosed, warm compresses and, in some cases, drainage may be necessary.
In conclusion, while the unilateral swelling of your son's right upper eyelid is likely related to his history of allergies, it is essential to seek professional medical advice for an accurate diagnosis and appropriate treatment. The combination of a thorough clinical evaluation and possibly some diagnostic tests will help clarify the situation and lead to effective management strategies.
