Lower abdominal pain?

Hello Doctor, after using the restroom today, I experienced a persistent sharp pain in my left lower abdomen while lying in bed.
The pain is localized, but when I sit up, it goes away.
However, when I lie down again, the sharp pain returns.
When I sit up, although the pain subsides, I still feel a dull sensation at the original pain site, as if it could become painful again at any moment.
What could this issue be? (I suspect it might be a stone, as I usually drink very little water, but there is no hematuria.)

Mr. Meng, 10~19 year old female. Ask Date: 2023/06/03

Dr. Du Shixi reply Urology

More Info

Understanding left lower abdominal pain can be complex, as it can arise from various underlying conditions. Based on your description, the pain you experience seems to be positional, intensifying when lying down and alleviating when sitting up. This pattern can provide valuable clues to potential causes.

Possible Causes of Left Lower Abdominal Pain
1. Musculoskeletal Issues: Given that the pain is exacerbated by certain positions, it could be related to musculoskeletal problems such as muscle strain or ligamentous issues. Activities that involve bending or twisting can lead to strain in the abdominal or back muscles, causing localized pain.

2. Gastrointestinal Conditions: Conditions such as diverticulitis, which involves inflammation of diverticula in the colon, can cause pain in the lower abdomen, particularly on the left side. Other gastrointestinal issues like irritable bowel syndrome (IBS) or constipation can also lead to discomfort and cramping sensations.

3. Urological Concerns: Since you mentioned a suspicion of kidney stones, it’s important to consider that while kidney stones can cause severe pain, they typically present with additional symptoms such as hematuria (blood in urine) or severe flank pain. However, if the stone is located in the ureter, it can cause referred pain to the lower abdomen.

4. Reproductive System Issues: In females, conditions such as ovarian cysts, ectopic pregnancy, or pelvic inflammatory disease can manifest as left lower abdominal pain. If you are female, it would be prudent to consider these possibilities, especially if there are any changes in your menstrual cycle or other gynecological symptoms.

5. Nerve Entrapment or Irritation: Sometimes, nerve issues can cause pain that is positional. Conditions like meralgia paresthetica, where the lateral femoral cutaneous nerve is compressed, can lead to pain that varies with position.

The Role of Anxiety
You mentioned that you are prone to anxiety, which can exacerbate the perception of pain. Anxiety can lead to muscle tension and increased sensitivity to pain, making it feel more intense. It’s crucial to address both the physical and psychological aspects of your health.

Recommendations
1. Hydration: Since you suspect kidney stones and mentioned low water intake, increasing your hydration is essential. Aim for at least 8-10 glasses of water daily to help prevent stone formation.

2. Dietary Considerations: A diet rich in fiber can help with gastrointestinal issues. Incorporate fruits, vegetables, and whole grains to promote regular bowel movements.

3. Medical Evaluation: Given the persistence and nature of your symptoms, it is advisable to consult a healthcare provider for a thorough evaluation. This may include imaging studies such as an ultrasound or CT scan to rule out any structural issues, especially concerning the kidneys or gastrointestinal tract.

4. Pain Management: Over-the-counter pain relief, such as NSAIDs, may help alleviate discomfort, but it’s essential to use them as directed and consult your doctor if pain persists.

5. Follow-Up: If initial tests do not reveal a cause, consider follow-up appointments to explore other potential diagnoses, including referrals to specialists such as a gastroenterologist or urologist.

In conclusion, while there are several potential causes for your left lower abdominal pain, a thorough medical evaluation is necessary to determine the exact cause and appropriate treatment. Don’t hesitate to seek medical attention, especially if your symptoms worsen or new symptoms arise.
