Should You Consider Penile Vein Ligation Surgery for Venous Leak? - Urology

Is penile venous stripping surgery recommended for venous leakage?

I do not have any other diseases, but I have been diagnosed with venous leakage issues by both Cheng Kung University Hospital and Dr.
Hsu Geng-Rong in Taipei.
I would like to address this problem.
Would you recommend Dr.
Hsu's penile vein ligation surgery? I have seen posts online about failed cases, and I am uncertain whether this surgery can effectively resolve the venous leakage issue or if I should proceed directly with penile prosthesis implantation.
Currently, I require a larger dosage of medication, which often makes me feel dizzy after taking it.
I am hoping for a solution and would like to know your opinion on whether I should consult Dr.
Hsu for this surgery.
I have heard that in some countries, vein ligation is not recognized, and they primarily perform penile prosthesis implantation, which is why I am hesitant about whether to first undergo penile vein ligation surgery or go straight for the penile prosthesis.

Dr. Xu Wencang reply Urology

You are only 24 years old; it's advisable to evaluate multiple options before making a decision.
Penile prosthesis implantation is irreversible, so it may be better to try other treatments first.

When considering penile vein ligation surgery for venous leak, it's essential to weigh the potential benefits against the risks and limitations of the procedure. Venous leak, or erectile dysfunction due to inadequate venous occlusion, can significantly affect a man's quality of life. The surgical option, which involves ligating or removing the problematic veins, aims to improve blood retention in the penis during an erection.

Firstly, it's crucial to understand that while penile vein ligation can be effective for some patients, it is not universally successful. The success rates can vary significantly based on individual anatomy, the severity of the venous leak, and the surgeon's experience. Reports of failed surgeries do exist, and these can stem from various factors, including improper patient selection or underlying conditions that were not adequately addressed.

In your case, since you have already undergone evaluations at reputable institutions, including checks by Dr. Hsu, it would be prudent to discuss your specific situation in detail with him. He can provide insights tailored to your condition, including the likelihood of success with vein ligation versus other options like penile prosthesis implantation.

Penile prosthesis surgery is often considered when other treatments, including medications, have failed. This option involves implanting a device that allows for an erection on demand, which can be a more reliable solution for many men. While it is a more invasive procedure, it has a high satisfaction rate among patients. However, it is essential to consider the recovery time, potential complications, and the irreversible nature of this option.

Regarding the concerns about medications causing dizziness, it is vital to communicate these side effects to your healthcare provider. They may be able to adjust your medication or explore alternative treatments that could alleviate your symptoms while still addressing the venous leak.

In terms of the acceptance of penile vein ligation in different countries, it is true that practices can vary. Some countries may favor penile prosthesis over vein ligation due to the latter's mixed success rates. This difference in approach can influence your decision, especially if you are considering traveling for treatment or seeking a second opinion.

Ultimately, the decision should be made collaboratively with your healthcare provider, considering your medical history, the severity of your condition, and your personal preferences. It may also be beneficial to seek a second opinion from another urologist who specializes in erectile dysfunction to explore all available options thoroughly.

In conclusion, while penile vein ligation surgery can be a viable option for some patients with venous leak, it is essential to have realistic expectations about its success and to consider all available alternatives, including penile prosthesis. Open communication with your healthcare provider will be key in determining the best course of action for your specific situation.
