Erectile dysfunction, engorgement, rigidity, corpus cavernosum, endurance?
Seven years ago, I sustained an injury to the lower right side of my penis and right testicle, resulting in bruising and hematoma.
Now, when I urinate, the stream deviates to the left.
I can still achieve an erection even without direct stimulation, but previously, my penis was straight with visible blood vessels.
Could this indicate nerve damage? I have difficulty exerting force at the base of the penis during urination, and I need to apply pressure with my fingers on the lower right side of the base to achieve an erection.
Is the injury affecting the blood vessels, connective tissue, nerves, veins, arteries, or the corpora cavernosa? Has the corpus spongiosum been affected? The corpora cavernosa seems to be intact since I can still achieve morning and nocturnal erections.
What treatment options are available? I have visited major hospitals and undergone ultrasound examinations, but the medications prescribed, including Corvitin F50 and Changchun Sustained Release Film-Coated Tablets 400mg, have not been effective.
I asked the doctor about vascular and nerve imaging, but they said it was unnecessary.
On June 5th, I received an injection of Laptops tadalafil 20mg/vial, which allowed me to achieve an erection for five hours, but I experienced pain on the right side of the base.
The erection gradually subsided by 4:40 PM.
Without the injection, I cannot achieve an erection, and I also experience pain at the right base during urination.
On June 8th, I was prescribed Pentoxifylline 400mg/tab (Fylin) twice daily for a total of 56 tablets, and Acetaminophen 500mg/tab (Societal) one tablet three times daily for a total of 9 tablets.
Is there an injury to the erectile tissue or is there venous leakage? Without the injection, I cannot achieve an erection.
I plan to have blood tests to check hormone levels.
I am considering whether medications like Weima Shupei's Xunli Ning or Cialis would be effective for treatment.

Dear Mr.
Weimashupei,
I apologize, but I cannot speculate on your condition from seven years ago.
I suggest you consider trying Viagra or Cialis.

Erectile dysfunction (ED) is a complex condition that can arise from various causes, including physical injuries, psychological factors, and underlying health issues. Based on your description, it seems that your ED may be related to a previous injury sustained seven years ago, which has affected the anatomy and function of your penis and possibly the surrounding structures.
Causes of Erectile Dysfunction
1. Physical Injury: The trauma you experienced to the base of the penis and the right testicle may have resulted in damage to the blood vessels, nerves, or erectile tissue (corpus cavernosum). This could lead to impaired blood flow, which is essential for achieving and maintaining an erection. If the corpus cavernosum or the surrounding structures were injured, it could explain the difficulty in achieving a firm erection without manual assistance.
2. Nerve Damage: The erectile process is heavily reliant on nerve function. If the injury affected the nerves that control blood flow to the penis, this could result in ED. The fact that you can achieve erections with injections indicates that the erectile tissue can still respond to stimulation, but the nerve signals may be compromised.
3. Vascular Issues: Conditions such as venous leakage, where blood flows into the penis but does not remain there due to faulty veins, can also contribute to ED. If you are experiencing pain in the right side of the base of the penis, this could indicate vascular complications.
4. Hormonal Factors: Hormonal imbalances can also play a role in ED. Low testosterone levels can affect libido and erectile function. It is wise to follow through with your plan to check hormone levels, as this can provide additional insights into your condition.
Symptoms of Erectile Dysfunction
- Difficulty achieving or maintaining an erection
- Pain during erections or ejaculation
- Changes in urinary stream or difficulty urinating
- Psychological distress related to sexual performance
Treatment Options
1. Medications: You mentioned trying medications like L-arginine and Pentoxifylline. While these can improve blood flow, they may not be effective for everyone, especially if there is significant vascular or nerve damage. PDE5 inhibitors (like Viagra or Cialis) are commonly prescribed for ED and may be worth discussing with your doctor, especially since you had a positive response to the injection therapy.
2. Injection Therapy: As you experienced with the injection of L-Arginine, this method can provide a temporary solution for achieving an erection. However, if pain persists, it may be necessary to reassess the injection technique or the specific medication used.
3. Vacuum Erection Devices: These devices can help draw blood into the penis and create an erection. They are non-invasive and can be used in conjunction with other treatments.
4. Surgery: In cases where there is significant anatomical damage or vascular issues, surgical options may be considered. This could involve penile implants or vascular surgery to improve blood flow.
5. Psychological Support: Since ED can also have psychological components, counseling or therapy may be beneficial, especially if anxiety or stress related to sexual performance is present.
6. Lifestyle Modifications: Maintaining a healthy lifestyle, including regular exercise, a balanced diet, and avoiding smoking and excessive alcohol, can improve overall vascular health and potentially alleviate some symptoms of ED.
Conclusion
It is crucial to have a thorough evaluation by a urologist who specializes in sexual medicine. They can perform necessary tests, including hormonal evaluations and possibly imaging studies, to determine the best course of action tailored to your specific situation. Open communication with your healthcare provider about your symptoms, concerns, and treatment responses will be essential in managing your erectile dysfunction effectively.
