Unexplained weight loss?
Hello Doctor, about six months ago during my health check-up, I weighed approximately 66 kilograms.
Over the past six months, I haven't made any significant changes to my diet or increased my physical activity.
My meals typically consist of two a day, lunch and dinner.
A few days ago, I weighed myself and found that my weight has dropped to 61 kilograms.
Recently, I've been experiencing some gastrointestinal discomfort, and my appetite has decreased; I feel full quite easily.
I often burp or pass gas, and on top of that, I've been under increased academic stress lately.
Doctor, what could be happening in my situation? Thank you for taking the time to respond.

Dr. Ye Qianyu reply Family Medicine
Hello! The weight loss may be due to gastritis leading to poor absorption.
Please also estimate your caloric intake to see if it is lower than your daily caloric needs; if so, this could contribute to weight loss.
Gastrointestinal discomfort and loss of appetite may be symptoms of gastritis.
Please observe for any signs such as black stools.
If you continue to feel unwell, please consult a gastroenterologist.

More Info
Unexplained weight loss can be concerning, especially when it occurs without any intentional changes in diet or exercise. In your case, a drop from 66 kg to 61 kg over a six-month period, combined with gastrointestinal discomfort and decreased appetite, warrants further investigation. Here are some potential causes and considerations regarding your health:
1. Gastrointestinal Issues: Your symptoms of feeling full quickly, frequent burping, and flatulence could indicate underlying gastrointestinal problems. Conditions such as gastritis, peptic ulcers, or even food intolerances (like lactose intolerance or gluten sensitivity) can lead to discomfort and changes in appetite. It’s essential to consider if you’ve experienced any changes in bowel habits or if you have any abdominal pain, as these could provide further clues.
2. Stress and Mental Health: You mentioned increased academic pressure, which can significantly impact both appetite and weight. Stress and anxiety can lead to changes in eating patterns, either causing a loss of appetite or leading to emotional eating. In some cases, stress can also exacerbate gastrointestinal symptoms, leading to a cycle of discomfort and further reduced food intake.
3. Metabolic or Endocrine Disorders: Unexplained weight loss can sometimes be attributed to metabolic or endocrine disorders, such as hyperthyroidism, diabetes, or adrenal insufficiency. These conditions can alter your metabolism and appetite, leading to weight changes. A thyroid panel and blood glucose levels can help rule out these issues.
4. Nutritional Deficiencies: If your diet consists of only two meals a day, it’s possible that you may not be getting adequate nutrition. This can lead to deficiencies that affect your overall health and metabolism. A lack of essential nutrients can also contribute to gastrointestinal issues and decreased appetite.
5. Infections or Chronic Illness: Sometimes, unexplained weight loss can be a sign of an underlying infection or chronic illness. Conditions such as tuberculosis, cancer, or chronic inflammatory diseases can lead to weight loss and gastrointestinal symptoms. While these are less common, they should not be overlooked, especially if symptoms persist.
Recommendations:
- Consult a Healthcare Professional: It’s crucial to discuss your symptoms with a healthcare provider. They may recommend blood tests, imaging studies, or referrals to specialists (like a gastroenterologist) to investigate further.
- Keep a Food Diary: Documenting what you eat and any symptoms you experience can provide valuable information for your healthcare provider. This can help identify any patterns or triggers related to your gastrointestinal discomfort.
- Manage Stress: Consider incorporating stress management techniques into your routine, such as mindfulness, meditation, or physical activity. These can help improve your overall well-being and may positively impact your appetite.
- Balanced Nutrition: Aim to ensure that your meals are balanced and nutritious. If you’re unsure about your dietary intake, consulting with a registered dietitian can help you create a meal plan that meets your needs without compromising your health.
- Monitor Symptoms: Keep track of any changes in your symptoms, appetite, or weight. If you notice significant changes or if your symptoms worsen, seek medical attention promptly.
In summary, while unexplained weight loss can be alarming, understanding the potential causes and seeking appropriate medical advice can help you address the underlying issues effectively. Your health is paramount, and taking proactive steps now can lead to better outcomes in the future.
