Monocular floaters?
Hello, doctor.
Whenever I relax my eyes or daydream, I notice an increase in floaters.
During my eye examination yesterday, I experienced the same thing while testing my visual field.
I mentioned this to the doctor, and when I lie down and daydream, I see something resembling a spinning fan.
However, the examining doctor said that my eyes appeared normal based on the report, and I also underwent OCT imaging.
The doctor suggested that the issue might not be with my eyes, but rather that the symptoms are manifesting in my eyes.
Did the doctor recommend which specialty I should consult for further examination? This issue has been troubling me for three months.

Hello: 1.
Neurologist: The increase in the appearance of floaters during relaxation or daydreaming may indicate a potential issue with the nervous system.
Consulting a neurologist can help determine if there are any neurological disorders or other vision-related problems.
2.
Psychosomatic Medicine Specialist: Sometimes, psychological factors such as stress or anxiety can affect perception and visual sensations.
Consulting a psychosomatic medicine specialist can help rule out these issues.
3.
Internist: Certain systemic diseases or medication side effects may also lead to visual abnormalities.
Consulting an internist can assess your overall health and identify any potential systemic issues.

Dear Patient,
Thank you for reaching out with your concerns regarding the floaters you have been experiencing. Floaters are small spots, lines, or cobweb-like shapes that drift through your field of vision. They are often more noticeable when looking at a plain background, such as a blue sky or a white wall. While floaters are common and usually harmless, they can sometimes indicate underlying issues that may require further evaluation.
From your description, it seems that you have been experiencing these floaters particularly when your eyes are relaxed or when you are daydreaming. You mentioned that during a recent eye examination, the doctor found your eyes to be normal, and an Optical Coherence Tomography (OCT) scan did not reveal any significant issues. This is reassuring, as it suggests that there may not be any serious underlying conditions affecting your vision.
However, it is important to understand when to seek further medical advice regarding floaters. Here are some key points to consider:
1. Sudden Increase in Floaters: If you notice a sudden increase in the number of floaters, especially if accompanied by flashes of light or a shadow in your peripheral vision, it could indicate a retinal tear or detachment. This is a medical emergency, and you should seek immediate attention from an eye care professional.
2. Changes in Vision: If you experience any changes in your vision, such as blurriness, loss of peripheral vision, or a curtain-like shadow over your field of vision, you should consult an eye specialist promptly.
3. Persistent Symptoms: Since you have been experiencing these symptoms for three months, and they are causing you distress, it may be beneficial to seek a second opinion or consult a specialist in retinal diseases. Sometimes, floaters can be associated with conditions like posterior vitreous detachment, which is common as we age but can also lead to complications.
4. Underlying Health Conditions: If you have any underlying health conditions, such as diabetes or hypertension, which can affect your eye health, it is crucial to monitor your symptoms closely and maintain regular check-ups with your healthcare provider.
5. Psychological Impact: It’s also worth noting that persistent visual disturbances can lead to anxiety or stress, which may exacerbate your perception of the floaters. If you find that your symptoms are affecting your quality of life, discussing these feelings with a healthcare professional may be beneficial.
In terms of next steps, I recommend that you consider scheduling an appointment with a retinal specialist who can perform a thorough examination of your eyes and provide more targeted insights into your symptoms. They may conduct additional tests to rule out any potential issues that might not have been evident during your previous examinations.
In conclusion, while floaters are often benign, your ongoing symptoms warrant further investigation to ensure that there are no underlying issues. It’s always better to err on the side of caution when it comes to your eye health. Please do not hesitate to reach out to a specialist for further evaluation, and take care of your overall well-being.
Best wishes for your health,
