Breastfeeding and Menstruation

Hello, I would like to ask about my situation.
I am currently about a year and a half postpartum and still breastfeeding.
My menstrual cycle has returned a few times, and it generally lasts for seven days.
Yesterday was the sixth day, and it seemed to have ended.
However, after having sexual intercourse yesterday, there was no bleeding.
Then, this afternoon, I suddenly noticed a little bit of bleeding.
Could this be due to incomplete menstruation or bleeding after intercourse?

Miss Zhou, 30~39 year old female. Ask Date: 2023/06/24

Dr. Cao Guotao reply Obstetrics and Gynecology

If there is still bleeding, it is recommended to see a gynecologist.

After giving birth, many women experience various changes in their menstrual cycles and bleeding patterns, especially if they are breastfeeding. It is quite common for postpartum bleeding, known as lochia, to continue for several weeks after delivery. This bleeding can vary in color, consistency, and amount, and it typically decreases over time. However, the return of menstruation can be influenced by several factors, including breastfeeding, hormonal changes, and individual health conditions.

In your case, since you are approximately a year and a half postpartum and still breastfeeding, it is important to note that breastfeeding can suppress ovulation and delay the return of regular menstrual cycles. This is due to the hormone prolactin, which is elevated during breastfeeding and can inhibit the release of hormones necessary for ovulation. As a result, some women may experience irregular periods or even amenorrhea (absence of menstruation) while breastfeeding.

Regarding the bleeding you experienced after sexual intercourse, there are a few possibilities to consider. First, it could be related to the menstrual cycle itself. If your period was just ending, it is possible that there was still some residual blood that was expelled during intercourse. Alternatively, if your menstrual cycle has just resumed, the bleeding could be the start of a new period.

Another possibility is that the bleeding could be due to physical irritation or trauma to the vaginal tissues during intercourse, especially if they are still healing from childbirth. The vaginal walls can be more sensitive and prone to bleeding after delivery, particularly if there was any tearing or if you had an episiotomy during childbirth.
If the bleeding is light and resolves quickly, it may not be a cause for concern. However, if you experience heavy bleeding, significant pain, or if the bleeding persists, it is advisable to consult with your healthcare provider. They can perform a thorough examination to rule out any underlying issues, such as infections, retained placental tissue, or other complications.

In summary, the bleeding you experienced could be related to your menstrual cycle or the physical effects of intercourse. It is essential to monitor the situation and seek medical advice if you have any concerns about your postpartum recovery or menstrual health. Regular check-ups with your healthcare provider can help ensure that any potential issues are addressed promptly and effectively.
