Preoperative medication cessation?

Hello, doctor.
Today I took the last of the 21 pills of my birth control pack and plan to stop taking them for 2 months.
I would like to ask you: 1.
If I have sexual intercourse, or even ejaculation, 2-3 days after stopping the pills, is there a possibility of pregnancy? 2.
After stopping the pills, as the ovaries resume ovulation, is it possible for my menstrual cycles to be irregular for 2-3 months?

CC, 20~29 year old female. Ask Date: 2023/07/03

Dr. Cao Guotao reply Obstetrics and Gynecology

When considering the discontinuation of birth control pills, it's important to understand how these hormonal contraceptives work and the potential effects of stopping them. Birth control pills primarily function by preventing ovulation, thickening cervical mucus to block sperm, and thinning the uterine lining to prevent implantation.
1. Risk of Pregnancy After Discontinuation: If you stop taking the birth control pill and engage in unprotected intercourse shortly after, there is a possibility of becoming pregnant. While the hormones from the pill can remain in your system for a short period, ovulation can resume relatively quickly after stopping the pill. In many cases, ovulation can occur as soon as a few days after the last pill is taken, especially if you have a shorter cycle. Therefore, if you have unprotected sex 2-3 days after stopping the pill, there is a risk of pregnancy, particularly if you have not yet had a withdrawal bleed (the period that occurs after stopping the pill). If you are concerned about the possibility of pregnancy, it may be wise to use a backup contraceptive method, such as condoms, during this time.

2. Menstrual Cycle Irregularities: After discontinuing the pill, it is common for women to experience some irregularities in their menstrual cycles. The body needs time to readjust to its natural hormonal rhythms, which can lead to variations in cycle length, flow, and symptoms. Some women may experience irregular periods for the first few months after stopping the pill, while others may find that their cycles return to normal relatively quickly. Factors such as age, overall health, and the duration for which the pill was taken can influence how quickly your menstrual cycle stabilizes.
It's also worth noting that some women may experience symptoms such as mood swings, acne, or changes in libido as their body readjusts to its natural hormonal state. These symptoms are typically temporary and should resolve as your body recalibrates.

In summary, if you are planning to stop taking the birth control pill, be aware of the potential for pregnancy shortly after discontinuation, especially if engaging in unprotected sex. Additionally, expect some irregularities in your menstrual cycle as your body transitions back to its natural hormonal balance. If you have specific concerns or experience significant irregularities or symptoms, it is advisable to consult with a healthcare provider for personalized guidance and support.
