Fell and hit the back of the head on the ground?

Hello, Dr.
Peng.
I accidentally slipped and fell at 5 PM the day before yesterday, hitting the back of my head without being able to protect it.
The next morning, I started to have phlegm.
I found online that others who experienced similar head trauma also had phlegm without any external injuries, but they were diagnosed with cerebrospinal fluid leaking into the oral cavity through the nasal passages.
So, I went to the hospital the day before yesterday, and the doctor arranged for a non-contrast head CT scan.
The results showed no blood clots, but I was diagnosed with sinusitis.
Is it possible that the CT scan images could be misinterpreted as sinusitis when they actually show cerebrospinal fluid leakage? I have been experiencing phlegm for two days now, and I have a slight headache, but I have no external injuries.
I would appreciate your clarification on this matter.
Thank you.

Hello Ms.
Chen: Based on your condition, it seems more like sinusitis.
Nasal tissue fluid would only occur in the case of a skull base fracture.

It sounds like you've been through a challenging experience after your fall, and it's understandable to have concerns about your symptoms. Let's break down your situation regarding the potential diagnosis of sinusitis and the implications of your recent head injury.

First, it's important to recognize that after a fall, especially one that involves a blow to the head, various symptoms can arise. You mentioned experiencing phlegm and a headache, which can be associated with sinusitis. Sinusitis is an inflammation of the sinus cavities, often caused by infections, allergies, or other factors. Symptoms typically include nasal congestion, facial pain or pressure, headache, and sometimes a cough or phlegm production.

In your case, the CT scan you underwent did not show any blood clots, which is a positive sign. However, the presence of sinusitis was noted. It's crucial to understand that the CT scan is designed to visualize the structures of the head, including the sinuses, and can help identify any abnormalities such as fluid accumulation or inflammation in the sinus cavities. The fact that your scan did not reveal any blood clots suggests that there is no acute intracranial hemorrhage, which is a serious concern following head trauma.

Regarding your worry about cerebrospinal fluid (CSF) leaking into the nasal cavity, this is a rare but serious condition that can occur after a head injury. It typically presents with clear fluid draining from the nose or ears, often accompanied by other symptoms such as a severe headache or changes in consciousness. If you were experiencing a CSF leak, you would likely have more pronounced symptoms than just phlegm and a headache. The CT scan would also typically show signs of a leak, such as fluid in the nasal cavity or other abnormal findings.

Given that your symptoms have persisted for a couple of days, it is advisable to monitor them closely. If you experience worsening headaches, fever, changes in vision, or any new neurological symptoms (such as confusion, weakness, or difficulty speaking), you should seek medical attention immediately. These could be signs of complications that require prompt evaluation.

In the meantime, managing your sinusitis symptoms can be beneficial. Over-the-counter decongestants, saline nasal sprays, and staying hydrated can help alleviate some discomfort. If your symptoms do not improve or worsen, follow up with your healthcare provider for further evaluation and potential treatment options, which may include antibiotics if a bacterial infection is suspected.

In summary, while your symptoms may be concerning, the absence of blood clots on your CT scan is reassuring. The diagnosis of sinusitis is plausible given your symptoms, and it’s essential to continue monitoring your condition. If you have any doubts or if your symptoms change, don’t hesitate to reach out to your healthcare provider for further guidance. Your health and peace of mind are paramount, and seeking clarity on your symptoms is always a wise decision.
