Alternating diarrhea and constipation?

Hello Doctor, I would like to ask a question.
I was previously diagnosed with moderate fatty liver disease, and since then I have started exercising.
However, since I began exercising, I have frequently experienced alternating constipation and diarrhea, and sometimes my stomach makes gurgling noises, but in the end, it only results in passing gas.
Occasionally, I suddenly feel abdominal pain.
Recently, I just completed my military service, and during the training period, I exercised more than I do now, but I only experienced slight constipation and had normal bowel movements on some days.
Given my current situation, is it possible that I have irritable bowel syndrome or colorectal cancer?

Alternating diarrhea and constipation can be quite distressing and may indicate an underlying gastrointestinal issue. Given your history of moderate fatty liver and recent lifestyle changes, it’s essential to consider several factors that could contribute to your symptoms.

Firstly, the condition you are describing could indeed be consistent with Irritable Bowel Syndrome (IBS), which is characterized by a combination of abdominal pain and altered bowel habits, including alternating diarrhea and constipation. IBS is often triggered by stress, dietary changes, and sometimes underlying gastrointestinal disorders. Since you mentioned that you have recently started exercising more and have experienced significant lifestyle changes, these factors could contribute to the onset of IBS symptoms.

Another possibility is that your symptoms could be related to dietary changes. A sudden increase in physical activity can lead to changes in your diet, which might include higher fiber intake or different food types that your body may not be accustomed to. If you have increased your fiber intake significantly, it can sometimes lead to bloating and gas, which may explain the gurgling sounds and the feeling of needing to pass gas without a bowel movement.

Additionally, the presence of alternating diarrhea and constipation can also be associated with other gastrointestinal conditions, such as inflammatory bowel disease (IBD) or even colorectal cancer, especially if there are other concerning symptoms like significant weight loss, blood in the stool, or persistent abdominal pain. However, these conditions typically present with additional symptoms that may not be present in your case.

Given your concerns, it is crucial to consult with a healthcare professional who can perform a thorough evaluation. This may include a detailed history, physical examination, and possibly further diagnostic tests such as blood tests, stool tests, or imaging studies to rule out any serious conditions. If IBS is suspected, management typically involves dietary modifications, stress management techniques, and sometimes medications to help regulate bowel movements.

In the meantime, consider keeping a food diary to track what you eat and how it correlates with your symptoms. This can help identify any potential food triggers. Additionally, maintaining a balanced diet, staying hydrated, and incorporating regular physical activity can help manage symptoms.

Lastly, while it’s understandable to be concerned about serious conditions like colorectal cancer, it’s essential to approach this with a balanced perspective. Most cases of alternating diarrhea and constipation are benign and can be managed effectively with lifestyle changes and dietary adjustments. However, if symptoms persist or worsen, seeking medical advice is crucial for appropriate diagnosis and treatment.
