After hemorrhoid surgery, the hemorrhoid ointment caused redness, swelling, and itching?
Hello, Doctor.
I would like to ask a question.
It has been a month since my hemorrhoid surgery, and recently I visited a dermatologist due to redness, swelling, and itching around the anal area.
The dermatologist indicated that the hemorrhoid cream (Le Yu Zhi) contains steroids, which can cause skin thinning and lead to a fungal infection if used for an extended period.
Improvement is expected only after discontinuing the medication (Le Yu Zhi).
However, the original surgeon believes it is just a drug rash and not a concern.
Could you please advise, Doctor Zhang, whether this is more likely a fungal infection or a drug rash? When can I consider the wounds (internal and external hemorrhoids) healed enough to stop the medication? Since the internal hemorrhoid wound is not visible, how can I assess its healing? Are there alternative creams I can use instead of Le Yu Zhi?

Dr. Zhang Shuhao reply Surgery
Hemorrhoid ointments such as Le Yu Zhi contain analgesic and corticosteroid components, which are suitable for short-term symptom relief before surgery, but are not appropriate for long-term use and are less suitable after surgery.
It is recommended to switch to an antihistamine cream or to maintain skin ventilation and cleanliness to reduce irritation and allow for natural healing.
For internal hemorrhoid wounds, there is no need to apply medication after one month post-surgery.

After undergoing hemorrhoid surgery, it's not uncommon to experience complications such as redness, swelling, and itching around the surgical site. These symptoms can be concerning, especially when they arise after using a topical medication like the one you mentioned, which contains steroids.
Firstly, it's essential to understand that corticosteroids, while effective in reducing inflammation and itching, can lead to skin thinning and increased susceptibility to infections, including fungal infections. This is particularly relevant in sensitive areas such as the anal region, where moisture and friction can create an environment conducive to fungal growth.
Given that your dermatologist has suggested that the redness and itching may be due to the prolonged use of the hemorrhoid cream (樂癒痔), it is crucial to follow their advice and discontinue the use of this medication. The symptoms you are experiencing could indeed be attributed to either a drug rash or a fungal infection. A drug rash typically presents as a generalized reaction to a medication, while a fungal infection may cause localized redness, itching, and sometimes a rash that appears scaly or flaky.
To determine whether your symptoms are due to a fungal infection or a drug rash, consider the following:
1. Duration and Nature of Symptoms: If the redness and itching persist or worsen after stopping the medication, it may indicate a fungal infection. Fungal infections often require antifungal treatment.
2. Appearance of the Rash: Fungal infections may present with a distinct appearance, such as scaling or a well-defined border. If you notice these characteristics, it would be prudent to consult your healthcare provider for a proper diagnosis.
3. Other Symptoms: If you experience additional symptoms such as discharge, increased pain, or fever, seek medical attention promptly.
As for the healing of your surgical site, the timeline for recovery can vary. Generally, hemorrhoid surgery wounds can take several weeks to heal, and internal hemorrhoid sites may be more challenging to assess. Signs that your wound is healing include a reduction in pain, swelling, and redness. If you notice persistent symptoms or if the area becomes increasingly painful, it is advisable to consult your surgeon or a healthcare professional.
Regarding alternative treatments, there are several options available that may help alleviate your symptoms without the side effects associated with steroids. Over-the-counter creams containing witch hazel or aloe vera can provide soothing relief. Additionally, sitz baths with warm water can help reduce discomfort and promote healing.
If you are looking for a replacement for the hemorrhoid cream, consider discussing with your healthcare provider about other topical treatments that do not contain steroids. Options may include creams with lidocaine for pain relief or other non-steroidal anti-inflammatory medications.
In conclusion, it is essential to monitor your symptoms closely and maintain open communication with your healthcare providers. If you suspect a fungal infection or if your symptoms do not improve after discontinuing the medication, seek further evaluation. Your health and comfort are paramount, and appropriate management can significantly enhance your recovery experience.
