Medication for medical abortion?

On July 17, I took RU486, and on July 19, I took the second dose.
On the night of July 19, I expelled the gestational sac and a large structure that should be the embryo.
On July 19, I can start taking ergometrine to promote uterine contractions and prevent heavy bleeding.
Today, I started experiencing allergies again, with persistent coughing and sneezing.
I usually take terfenadine for relief.
I would like to know if ergometrine will interact with terfenadine and affect the expulsion of lochia.

Xinhan, 10~19 year old female. Ask Date: 2023/07/21

After a medical abortion, the use of Ergot Alkaloids, such as Ergometrine Maleate, is often prescribed to help stimulate uterine contractions and reduce the risk of excessive bleeding. However, it is essential to consider potential interactions with other medications, particularly antihistamines like Fexofenadine (the active ingredient in Allegra) that you mentioned.

Ergot Alkaloids, including Ergometrine, work primarily by stimulating the smooth muscles of the uterus, promoting contractions. This can be particularly beneficial after a medical abortion to ensure that the uterus expels any remaining tissue and to minimize the risk of hemorrhage. However, these medications can also have side effects, including gastrointestinal discomfort, headaches, and dizziness, as well as more serious cardiovascular effects like hypertension.

On the other hand, antihistamines such as Fexofenadine are commonly used to relieve allergy symptoms, including sneezing, runny nose, and itching. They work by blocking the action of histamine, a substance in the body that causes allergic symptoms. Generally, antihistamines do not have a direct interaction with Ergot Alkaloids. However, there are a few considerations to keep in mind:
1. Potential Side Effects: Both Ergot Alkaloids and antihistamines can cause dizziness and fatigue. When taken together, these side effects may be amplified, leading to increased discomfort or impaired functioning.

2. Uterine Contractions: While antihistamines do not directly affect uterine contractions, they can cause some degree of sedation. If you are experiencing significant drowsiness or dizziness from the antihistamine, it may be more challenging to monitor any side effects from the Ergot Alkaloid.

3. Monitoring Symptoms: After a medical abortion, it is crucial to monitor your symptoms closely. If you experience heavy bleeding, severe pain, or any other concerning symptoms, you should seek medical attention immediately. The combination of medications should not interfere with the expulsion of lochia (post-abortion discharge), but if you have concerns about your recovery, it is best to consult your healthcare provider.

4. Consult Your Doctor: It is always advisable to discuss any new medications or changes in your health status with your healthcare provider, especially after a medical procedure like an abortion. They can provide personalized advice based on your medical history and current medications.

In summary, while Ergot Alkaloids and antihistamines like Fexofenadine do not have a known direct interaction that would significantly impact the expulsion of lochia after a medical abortion, caution should be exercised due to the potential for increased side effects. Always consult with your healthcare provider for tailored advice and to ensure a safe recovery process.
