Post-Surgery Concerns: Healing Issues After Anal Fistula Surgery - Surgery

Post-anal fistula surgery

Hello Doctor: I underwent anal fistula surgery in mid-June, with the location approximately at the 6 o'clock position of the anus.
The wound has not fully healed yet, but I recently noticed a small abrasion in the area that had previously healed after the surgery.
It hurts when I touch the area while cleaning the anus or when it comes into contact with water.
Is this normal, or could the abrasion be due to activity? I would appreciate your assistance in answering this.
Thank you, Doctor!

Niming, 20~29 year old female. Ask Date: 2023/08/04

If there is no redness, swelling, heat, pain, or purulent discharge, then it should be fine.
After surgery, the healing skin is more fragile, so it is possible for the skin to break due to activity.
You can apply ointment and observe the area.
If the wound continues to not heal, you should consult the surgeon to check for any recurrence.

More Info

After undergoing anal fistula surgery, it is not uncommon to experience concerns regarding the healing process. Your situation, where you have noticed a small area of skin breakdown at the surgical site, is something that many patients may encounter during their recovery. Here’s a detailed response to your concerns, along with some additional information regarding post-surgical care and healing.

Firstly, it is important to understand that the healing process after anal fistula surgery can vary significantly from person to person. The surgical site is sensitive, and it is not unusual for patients to experience discomfort, pain, or even minor skin breakdown as they recover. The fact that you are experiencing pain when touching the area or when it comes into contact with water suggests that the tissue is still healing and may be sensitive.
The small area of broken skin you mentioned could be attributed to several factors, including:
1. Activity Level: Increased physical activity or straining during bowel movements can put additional stress on the surgical site, potentially leading to irritation or breakdown of the skin. It is advisable to avoid heavy lifting or strenuous activities until you receive clearance from your surgeon.

2. Hygiene Practices: While maintaining cleanliness is crucial, excessive scrubbing or harsh cleaning methods can irritate the area. Gentle cleansing with mild soap and water, followed by patting the area dry, is recommended. Avoid using any harsh chemicals or scrubs that could exacerbate irritation.

3. Moisture and Healing: The anal region is prone to moisture, which can affect healing. Keeping the area dry and clean is essential. You might consider using a barrier cream or ointment recommended by your healthcare provider to protect the skin and promote healing.

4. Diet and Bowel Movements: Ensuring that you have a soft stool can help minimize straining during bowel movements, which can further irritate the surgical site. A diet high in fiber, adequate hydration, and possibly the use of stool softeners can be beneficial.

Regarding your specific question about whether this situation is normal: while some discomfort and minor skin breakdown can occur, it is essential to monitor the area closely. If the pain worsens, if you notice increased redness, swelling, or discharge, or if the area does not improve, it is crucial to contact your surgeon or healthcare provider. They may want to evaluate the site to rule out any complications, such as infection or improper healing.

In terms of recovery time, healing from anal fistula surgery can take several weeks to months, depending on the extent of the surgery and individual factors such as overall health and adherence to post-operative care instructions. Regular follow-up appointments with your surgeon are important to assess healing progress and address any concerns.

In summary, while some discomfort and minor skin breakdown can be part of the healing process after anal fistula surgery, it is essential to maintain proper hygiene, avoid excessive strain, and monitor the area for any signs of complications. If you have any doubts or if your symptoms persist, do not hesitate to reach out to your healthcare provider for further evaluation and guidance. Your recovery is important, and addressing any concerns promptly can help ensure a smoother healing process.
