Urinary issues
I would like to ask if straining while urinating can have negative effects.
I currently feel the urge to urinate even after finishing.
A doctor at National Taiwan University mentioned that I have mild benign prostatic hyperplasia.
Is it really possible to develop prostate enlargement at my age, or could it be a result of frequently straining during urination?
A Zhi, 20~29 year old female. Ask Date: 2004/01/05
Dr. Liu Jianting reply Urology
Please! Don't overthink it, just let urination happen naturally.
However, if you experience pain while urinating, you should see a doctor for an examination.
It's hard to believe that at 20 years old, you could have benign prostatic hyperplasia!

More Info
Straining during urination can indeed have negative implications for your urinary health, particularly as it relates to prostate health. It is not uncommon for men, especially as they age, to experience benign prostatic hyperplasia (BPH), which is an enlargement of the prostate gland. This condition can lead to various urinary symptoms, including difficulty starting urination, a weak urine stream, and the sensation of incomplete bladder emptying, which seems to be what you are experiencing.
When you strain to urinate, you may inadvertently increase pressure in the bladder and urethra, which can lead to further complications. Straining can cause or exacerbate urinary retention, where the bladder does not empty completely. This can lead to urinary tract infections (UTIs) and bladder damage over time. Additionally, chronic straining can put stress on the pelvic floor muscles and potentially lead to pelvic floor dysfunction, which can cause further urinary issues.
Regarding your concern about prostate enlargement, it is indeed a common condition as men age. The prostate gland typically begins to enlarge around the age of 40, and by the age of 60, a significant number of men will experience some degree of BPH. While the exact cause of BPH is not fully understood, hormonal changes that occur with aging are believed to play a significant role. The enlargement of the prostate can compress the urethra, leading to the symptoms you are describing.
It is essential to differentiate between symptoms caused by BPH and those that may arise from straining during urination. While straining might not directly cause prostate enlargement, it can worsen the symptoms associated with it. Therefore, it is crucial to address both the straining and the underlying prostate condition.
Here are some recommendations to consider:
1. Consult a Urologist: Since you have already been diagnosed with some degree of prostate enlargement, it would be beneficial to consult a urologist who can provide a thorough evaluation and discuss treatment options. Treatments may include lifestyle changes, medications, or, in more severe cases, surgical interventions.
2. Avoid Straining: Focus on relaxing your pelvic floor muscles while urinating. This can help reduce the need to strain and may improve your urinary symptoms. Techniques such as deep breathing and mindfulness can assist in this process.
3. Hydration: Ensure you are adequately hydrated, as this can help with urine flow. However, avoid excessive caffeine and alcohol, as they can irritate the bladder and worsen urinary symptoms.
4. Pelvic Floor Exercises: Engaging in pelvic floor exercises (Kegel exercises) can strengthen the muscles involved in urination and may help alleviate some symptoms. However, it is essential to perform these exercises correctly, so consider consulting a physical therapist specializing in pelvic health.
5. Monitor Symptoms: Keep track of your urinary symptoms, including frequency, urgency, and any discomfort. This information can be valuable for your healthcare provider in determining the best course of action.
In summary, while straining during urination can lead to complications, it is also essential to address the underlying issue of prostate health. With appropriate medical guidance and lifestyle modifications, you can manage your symptoms effectively and maintain a better quality of life.
