Duration of Retainer Wear After Orthodontic Treatment
Hello, Doctor.
I am currently four days post-orthodontic treatment.
During the treatment, I had four teeth extracted and bone screws were used.
Now, my doctor has provided me with a clear retainer.
My concern is that when I wear the retainer, I cannot fully close my mouth, and there is a significant gap between my lips.
I also have difficulty articulating my speech.
I would like to know if, in two weeks, I will be able to wear the retainer for a full day (including while sleeping) for fifteen to sixteen hours.
Additionally, I would like to inquire if there are alternative types of retainers that would allow me to close my mouth properly without my lips protruding.
Thank you, Doctor.

Hello: It has been four days since your orthodontic treatment.
I strongly recommend that you follow the doctor's instructions and wear the retainer.
If it affects your speech or appearance, you can discuss with your doctor the possibility of switching to a different type of retainer that is fixed on the lingual side (although it also has its drawbacks).

More Info
After completing orthodontic treatment, it's common to feel discomfort when wearing retainers, especially if you've had extractions or used bone screws during your treatment. Your experience of not being able to close your lips completely while wearing a clear retainer is not unusual, and it can be frustrating as it affects both your comfort and speech.
Duration of Retainer Wear
Typically, orthodontists recommend wearing retainers full-time for a certain period after braces are removed, often around 3 to 6 months, followed by a transition to nighttime wear. In your case, since you are on day four of wearing your retainer, it’s essential to follow your orthodontist's specific instructions regarding the duration of wear. If your orthodontist has suggested wearing the retainer for 15 to 16 hours a day, you should aim to adhere to that guideline, gradually increasing your comfort level with the retainer.
Comfort Solutions
1. Gradual Adjustment: It may take some time for your mouth to adjust to the retainer. If you find it uncomfortable, try wearing it for shorter periods initially and gradually increase the duration as you become more accustomed to it.
2. Speech Practice: If speech is affected, practice speaking with the retainer in. This can help your mouth adjust to the retainer and improve your articulation over time.
3. Consult Your Orthodontist: If the retainer is causing significant discomfort or if you feel it is not fitting correctly, it’s crucial to return to your orthodontist. They can assess the fit and make necessary adjustments. Sometimes, a different type of retainer might be more suitable for your needs.
4. Alternative Retainers: There are various types of retainers available, including fixed retainers (bonded behind the teeth) and different designs of removable retainers. Discuss with your orthodontist the possibility of switching to a different type that might allow for better comfort and lip closure.
5. Oral Hygiene: Maintaining good oral hygiene while wearing a retainer is essential. Clean your retainer regularly to prevent any buildup that could cause discomfort or bad breath.
6. Pain Relief: If you experience soreness, over-the-counter pain relief medications may help. However, consult with your orthodontist before taking any medication.
Conclusion
In summary, while it’s normal to experience some discomfort and difficulty with speech when first wearing a retainer, it’s important to communicate openly with your orthodontist about your concerns. They can provide tailored advice and adjustments to ensure that your retainer fits well and serves its purpose effectively. Remember, the goal of wearing a retainer is to maintain the results of your orthodontic treatment, so finding a comfortable solution is key to your long-term dental health.
