Duration of Retainer Wear After Orthodontic Treatment

Hello Doctor, I am currently four days post-orthodontic treatment.
During the treatment, I had four teeth extracted and bone screws were used.
Now, my doctor has instructed me to wear a clear retainer.
My concern is that when I wear the retainer, I cannot fully close my mouth, and there is a significant gap between my lips, which affects my speech and pronunciation.
I would like to know if it is possible for me to wear the retainer for fifteen to sixteen hours a day, including during sleep, in two weeks.
Additionally, I would like to ask if there are alternative types of retainers that would allow me to close my mouth properly without my lips protruding.
Thank you, Doctor.

Hello: It's been four days since your orthodontic treatment.
I strongly recommend that you follow the doctor's instructions and wear the retainer.
If it affects your speech or appearance, you can discuss with your doctor the possibility of switching to a different type of retainer that is fixed on the lingual side (though it also has its drawbacks).
The solid form of the retainer should be worn in the mouth 24 hours a day.

After completing orthodontic treatment, it's common to be prescribed a retainer to maintain the new position of your teeth. Retainers play a crucial role in preventing teeth from shifting back to their original positions, which is a common occurrence after braces are removed. Your concerns about the fit and comfort of your retainer are valid, and it's essential to address them to ensure effective treatment.

Firstly, it's important to understand that retainers, especially clear aligners, may feel a bit uncomfortable initially. They can create a sensation of pressure as they hold your teeth in their new positions. However, if you are experiencing significant discomfort, such as an inability to close your lips or difficulty speaking, this could indicate that the retainer may not fit properly. It's advisable to return to your orthodontist for an evaluation. They can check the fit of your retainer and make any necessary adjustments. Sometimes, a slight modification can make a significant difference in comfort.

Regarding your question about wearing the retainer for 15 to 16 hours a day, this is typically a reasonable expectation after the initial adjustment period. Most orthodontists recommend wearing retainers full-time for a few months following the removal of braces, gradually transitioning to nighttime wear only. However, the specific duration can vary based on individual cases, so it’s best to follow your orthodontist's instructions. If you are struggling with the retainer during the day, you might consider wearing it primarily at night initially, as long as you are still meeting the recommended wear time.

As for alternative retainer options, there are several types available. If the clear retainer is causing issues, you might want to discuss with your orthodontist the possibility of using a Hawley retainer, which is made of a combination of plastic and metal. This type of retainer is adjustable and can sometimes provide a better fit, allowing for more comfort and easier speech. Your orthodontist can help you determine the best option based on your specific needs and preferences.

In summary, while it is normal to experience some discomfort with a new retainer, significant issues like the inability to close your lips or speak clearly should be addressed with your orthodontist. They can assess the fit and make adjustments or suggest alternative retainer types that may be more comfortable for you. Remember, the goal of wearing a retainer is to maintain the beautiful results of your orthodontic treatment, so it's worth taking the time to ensure that it fits well and is comfortable.
