Perianal mass

Hello doctor, in the past day or two, I have felt a lump near my anus.
It feels soft, is about 1 centimeter in size, appears to be a normal skin color, and is not painful to the touch.
I have a slight sensation of pressure when having a bowel movement, but there is no bleeding or blood in the stool.
When sitting at the computer, I hardly feel it.
I'm unsure how to proceed with this.

Lin, 10~19 year old female. Ask Date: 2023/08/21

Dr. Zhang Shuhao reply Surgery

A sudden appearance of a soft, painless lump near the anus, along with a slight sensation of pressure during bowel movements, is more likely to be due to swollen hemorrhoids, which can result from bleeding and inflammation of the subcutaneous hemorrhoidal tissue.
It is advisable to increase fluid intake to prevent constipation and to take warm sitz baths to help reduce swelling.
If symptoms persist, it is recommended to consult a colorectal specialist for examination.

More Info

When you discover a soft lump near the anus, it can understandably cause concern. However, it’s important to approach the situation with a calm mindset and gather as much information as possible to determine the appropriate course of action.
Based on your description, the lump is soft, approximately 1 cm in size, skin-colored, and not painful. It’s also noteworthy that you do not experience any bleeding or blood in your stool, which are significant symptoms that could indicate more serious conditions.
There are several potential causes for a soft lump in that area, and understanding them can help you decide how to proceed:
1. Hemorrhoids: One of the most common causes of lumps near the anus is hemorrhoids, which are swollen veins in the lower rectum and anus. They can be internal or external. External hemorrhoids can appear as soft lumps and may not always be painful unless they become thrombosed (clotted).

2. Skin Tags: These are benign growths that can occur around the anal area, often as a result of previous irritation or friction. They are typically soft and painless.

3. Abscess: An anal abscess is a collection of pus that can occur due to infection. While these are usually painful, some may present as soft lumps without significant pain initially.

4. Cysts: Sebaceous cysts can form in various areas of the body, including near the anus. They are generally harmless and can feel soft to the touch.

5. Fistulas: An anal fistula is a small tunnel that forms between the end of the bowel and the skin near the anus. This condition often follows an abscess and may present as a lump.

Given that the lump is not painful and you are not experiencing any alarming symptoms, it may not require immediate medical intervention. However, it is essential to monitor the lump for any changes in size, shape, or symptoms. Here are some steps you can take:
- Maintain Hygiene: Keep the area clean and dry. Gentle washing with mild soap and water can help prevent irritation.

- Avoid Straining: If you experience any discomfort during bowel movements, try to avoid straining. A diet high in fiber and adequate hydration can help ensure softer stools.

- Warm Compresses: Applying a warm compress to the area may provide relief if there is any discomfort.

- Over-the-Counter Treatments: If you suspect hemorrhoids, over-the-counter creams or ointments may help alleviate symptoms.

- Monitor Symptoms: Keep an eye on the lump. If it grows, becomes painful, or if you start experiencing bleeding or changes in bowel habits, it’s crucial to seek medical attention.

- Consult a Healthcare Provider: If the lump persists for more than a couple of weeks or if you have any concerns, it’s advisable to consult a healthcare provider. They can perform a physical examination and may recommend further tests, such as an ultrasound or a colonoscopy, if necessary.

In summary, while a soft lump near the anus can be concerning, many causes are benign. Monitoring the lump and maintaining good hygiene are essential steps. However, do not hesitate to seek medical advice if you notice any changes or if you have any concerns about your health. Early intervention can help address any potential issues before they become more serious.
