Unexplained vaginal bleeding?

Recently, there has been no sexual activity.
Today, while wearing jeans, I suddenly squatted down and experienced a vague feeling of foreign body sensation and sharp pain in the vagina.
Later in the evening, when I went to the bathroom and wiped the vaginal area with toilet paper, I noticed a small amount of pink blood.
The bleeding has not stopped, and there are no visible wounds on the outside of the vaginal opening, yet there is still pain and bleeding when wiped.

Dr. Lin Xinwang reply Obstetrics and Gynecology

Hello, Kang: Abnormal vaginal bleeding during ovulation, if it occurs for a short duration of 1 to 2 days, is considered a normal phenomenon.
However, if it is not during this time, a clinic visit is necessary to check for possible infections or endocrine disorders and other causes.

More Info

Unexpected vaginal bleeding can be concerning, especially when it occurs outside of your regular menstrual cycle. Based on your description, it sounds like you experienced a sudden onset of bleeding accompanied by discomfort, which can be attributed to several potential causes.
Firstly, it’s important to note that vaginal bleeding can arise from various sources within the reproductive system. Since you mentioned that you have not engaged in sexual intercourse recently, we can rule out pregnancy-related issues. However, there are still several other possibilities to consider:
1. Vaginal or Cervical Irritation: Activities such as vigorous exercise, wearing tight clothing (like jeans), or even certain movements (like squatting) can sometimes cause irritation to the vaginal walls or the cervix. This irritation can lead to minor bleeding, especially if there is any underlying inflammation or infection.

2. Infections: Conditions such as bacterial vaginosis or yeast infections can cause inflammation and irritation of the vaginal tissues, leading to bleeding. If you have experienced any unusual discharge, odor, or itching, these could be signs of an infection.

3. Hormonal Imbalances: Hormonal fluctuations can lead to unexpected bleeding. Conditions such as polycystic ovary syndrome (PCOS) or thyroid disorders can disrupt your menstrual cycle and cause irregular bleeding.

4. Trauma or Injury: Even if you do not see any visible wounds, minor trauma to the vaginal area can occur and may not always be apparent. This could be due to friction from clothing or even from activities like vigorous exercise.

5. Cervical Erosion or Polyps: Sometimes, the cervix can become eroded or develop polyps, which can cause bleeding, especially after physical activity. These conditions are usually benign but should be evaluated by a healthcare provider.

6. Other Medical Conditions: Conditions such as endometriosis or fibroids can also lead to unexpected bleeding. These conditions often come with additional symptoms, such as pelvic pain or heavy menstrual bleeding.

Given your symptoms of pinkish blood and persistent bleeding, it is advisable to seek medical attention. A healthcare provider can perform a thorough examination, which may include a pelvic exam, to determine the cause of the bleeding. They may also recommend tests such as a Pap smear or ultrasound if necessary.

In the meantime, here are some steps you can take:
- Avoid Irritation: Wear loose-fitting clothing and avoid activities that may exacerbate the discomfort.

- Monitor Symptoms: Keep track of any additional symptoms, such as pain, changes in discharge, or the amount of bleeding.

- Stay Hydrated and Rest: Ensure you are well-hydrated and allow your body to rest as needed.

It’s important to communicate openly with your healthcare provider about your symptoms, including any recent activities or changes in your health. This information can help them make a more accurate diagnosis and provide appropriate treatment. Remember, while minor bleeding can sometimes resolve on its own, persistent or heavy bleeding warrants prompt medical evaluation to rule out any serious conditions.
