The discharge is brown and has an odor?

Hello doctor, my menstrual period started on August 15th, and now (on the 30th) I am still experiencing brown discharge with a fishy odor.
It flows out whenever I exert myself, to the point that it drips down.
After a while, when I exert myself again, it continues to flow.
Currently, I do not have any itching or discomfort, but I immediately go to the bathroom to clean up when I feel it coming out.
Could this be related to sexual intercourse? The odor is quite bothersome.
A year ago, I had an issue with a vaginal infection, which resulted in yellow-green discharge, and I saw a doctor to resolve it.
However, this time it is different, as it is brown.
I would like to know what might be causing this.

Yu, 20~29 year old female. Ask Date: 2023/08/30

Dr. Lin Xinwang reply Obstetrics and Gynecology

Hello, Yu: Brown discharge indicates a small amount of bleeding from the vagina or uterus.
Over time, it can turn brown.
This may be due to an infection or abnormal uterine bleeding, and it is necessary to seek medical evaluation to confirm the cause and receive treatment.

Understanding brown discharge with an odor can be concerning, especially when it deviates from what is typically expected during your menstrual cycle. Based on your description, it seems that you are experiencing a brown discharge that has a fishy odor, which has persisted since your last menstrual period on August 15th. This situation raises several important considerations regarding potential causes and concerns.

Firstly, brown discharge is often associated with the end of a menstrual period, as it can be old blood that is taking longer to exit the body. However, the presence of an unusual odor, especially one that is fishy, can indicate an underlying issue that may require medical attention. The odor you describe could suggest a bacterial infection, such as bacterial vaginosis (BV), which is characterized by an imbalance of normal vaginal flora. BV can lead to a discharge that is gray or brown and has a strong, fishy smell, particularly after intercourse.

Another possibility is that the discharge could be related to retained menstrual blood, especially if you have recently had a period. If blood is not expelled completely, it can oxidize and turn brown, leading to a discharge that may have an odor. However, the fact that you are experiencing this discharge for an extended period (since mid-August) raises concerns that it may not simply be residual menstrual blood.

Additionally, sexually transmitted infections (STIs) such as chlamydia or gonorrhea can also cause abnormal discharge and may present with a foul odor. Although you mentioned that you do not currently have itching or discomfort, some STIs can be asymptomatic, making it crucial to consider this possibility, especially if you have had unprotected sexual intercourse.

Given your history of a previous infection that resulted in yellow-green discharge, it is essential to be vigilant about any changes in your vaginal health. The recurrence of abnormal discharge, particularly with an odor, warrants a thorough evaluation by a healthcare professional. A gynecological examination can help determine the cause of your symptoms, and laboratory tests can identify any infections or imbalances that may be present.

In summary, while brown discharge can sometimes be a normal part of the menstrual cycle, the combination of prolonged duration and a fishy odor suggests that it may be indicative of an underlying issue, such as bacterial vaginosis or an STI. It is advisable to schedule an appointment with your healthcare provider for a comprehensive evaluation. They can perform the necessary tests and provide appropriate treatment options to address your concerns and restore your vaginal health. Remember, early intervention is key to managing any potential infections and ensuring your overall well-being.
