Diplopia

Hello Doctor, I am currently experiencing double vision, specifically with the left upper and right lower images overlapping (sometimes accompanied by slight throbbing pain in the left forehead).
I have visited an ophthalmology clinic and undergone CT and MRI scans at Kaohsiung Veterans General Hospital, and the doctors indicated that everything is normal.
I am currently taking "Chuang Xue Mai" enteric-coated micro-particle capsules (100 mg) and "Hsin Hui" Pei Duo Huo Xing Bi sugar-coated tablets, but there has been no improvement! (The double vision has persisted for about a month.) I would like to ask the doctor what I should do next.

Xin, 30~39 year old female. Ask Date: 2023/09/01

Dr. Gao Pilin reply Ophthalmology

Hello: Sometimes, diplopia may persist for several weeks to months, or even longer, for the following reasons: 1.
The causes of diplopia can be various eye issues, such as ocular muscle problems or optic nerve issues.
The recovery time will vary depending on the underlying cause.
2.
The severity of the diplopia can also affect the recovery time.
Mild diplopia may resolve more quickly, while severe cases may take longer.
Since it has been a month, it is possible that it may take even more time.
Please be patient and follow your physician's recommendations, and return for follow-up appointments as scheduled.

Double vision, or diplopia, can be a perplexing and distressing condition, often resulting from various underlying causes. Given your symptoms of left upper and right lower overlap, along with occasional pain in the left forehead area, it is crucial to explore potential causes and management strategies.

Understanding the Causes of Double Vision
1. Neurological Issues: Diplopia can arise from neurological conditions affecting the cranial nerves responsible for eye movement. Conditions such as multiple sclerosis, myasthenia gravis, or even a stroke can lead to misalignment of the eyes, resulting in double vision.

2. Ocular Conditions: Problems with the eye muscles, such as strabismus (misalignment of the eyes), can also cause diplopia. Additionally, conditions like thyroid eye disease can lead to swelling of the eye muscles, affecting their function.

3. Trauma: Any recent head injury or trauma to the eye area can lead to double vision due to damage to the eye muscles or nerves.

4. Systemic Conditions: Diabetes and hypertension can lead to vascular issues affecting the eye muscles or nerves, resulting in diplopia.

5. Medications: Certain medications can cause side effects that include double vision. It's essential to review any recent changes in medication with your healthcare provider.

Management Strategies
1. Follow-Up with Specialists: Since you have already undergone imaging studies (CT and MRI) that returned normal results, it may be beneficial to consult with a neurologist or an ophthalmologist specializing in neuro-ophthalmology. They can provide a more in-depth evaluation of your symptoms and may recommend additional tests, such as blood tests or visual field tests.

2. Vision Therapy: Depending on the underlying cause, vision therapy may be beneficial. This therapy involves exercises designed to improve coordination and strength of the eye muscles, which can help alleviate diplopia.

3. Prism Glasses: If the double vision is due to misalignment of the eyes, prism glasses can be prescribed. These glasses help align the images seen by each eye, reducing the perception of double vision.

4. Medication Adjustment: If you suspect that your current medications may be contributing to your symptoms, discuss this with your healthcare provider. They may consider adjusting your medications or exploring alternative treatments.

5. Surgical Options: In some cases, surgical intervention may be necessary to correct the alignment of the eyes, especially if the diplopia is persistent and significantly affects your quality of life.

6. Pain Management: For the forehead pain you are experiencing, consider discussing pain management options with your doctor. This could include over-the-counter pain relievers or other medications that target nerve pain.

Lifestyle Considerations
- Eye Rest: Ensure you are giving your eyes adequate rest, especially if you are spending long hours in front of screens.

- Hydration and Nutrition: Maintain a balanced diet and stay hydrated, as overall health can impact eye health.

- Stress Management: Stress can exacerbate many conditions, including those affecting vision. Consider relaxation techniques or mindfulness practices.

Conclusion
Double vision can be a complex condition with various potential causes. Since your imaging studies have returned normal, it is essential to continue working closely with your healthcare providers to explore further evaluations and management options. Keeping an open line of communication with your doctors about your symptoms and any changes you experience will be crucial in finding an effective solution. Remember, while diplopia can be concerning, many underlying causes are treatable, and with the right approach, improvement is often achievable.
