Anal itching

Hello, Doctor: I would like to ask you about occasional itching around my anus (sometimes occurring after my menstrual period).
Before having my daughter, I had issues with constipation (often experiencing difficulty in bowel movements, feeling the urge but unable to pass stool, or managing to pass stool intermittently but not feeling completely relieved).
There is a small polyp at the anal opening (which was present before pregnancy and has been there for three years post-delivery).
Recently (about four days ago), I have been experiencing anal itching along with a rash resembling eczema, and for the past two days, I have also felt itching in my groin area.
I often feel moisture around my anus, but I always wipe dry after using the restroom.
However, I feel embarrassed to seek medical attention (I have heard about colleagues of my husband who had to undergo internal examinations for hemorrhoids), so I have been using a topical ointment prescribed for my mother-in-law's skin itching as a temporary solution.
Is this acceptable? What could this condition be? I would appreciate your clarification.
Thank you.

Hello: Based on your description, it appears to be a chronic anal fissure (caused by constipation).
Please seek a consultation with a colorectal surgeon for diagnosis and treatment as soon as possible.
You may refer to our hospital's educational website for information on constipation and anal itching, which may be helpful.

Anal itching, also known as pruritus ani, is a common condition that can be caused by a variety of factors. Understanding the underlying causes, symptoms, and treatment options is essential for effective management.

Causes of Anal Itching
1. Hygiene Issues: Poor hygiene can lead to irritation and itching. Conversely, excessive cleaning or the use of harsh soaps can also irritate the sensitive skin around the anus.

2. Skin Conditions: Conditions such as eczema, psoriasis, or fungal infections can cause itching. The presence of a rash or similar symptoms, as you described, may indicate a dermatological issue.

3. Hemorrhoids: Swollen blood vessels in the rectal area can lead to itching, especially if they are inflamed or bleeding.

4. Dietary Factors: Certain foods, such as spicy foods, caffeine, and alcohol, can irritate the anal area and lead to itching.

5. Infections: Bacterial or yeast infections, as well as sexually transmitted infections, can cause anal itching.

6. Worms: Pinworms, especially in children, can cause significant itching around the anus, particularly at night.

7. Medical Conditions: Conditions like diabetes or inflammatory bowel disease can also lead to anal itching.

Symptoms
The primary symptom is itching around the anal area, which can be accompanied by:
- Redness or irritation
- A burning sensation
- Discomfort during bowel movements
- Rash or bumps in the anal area
- Moisture or discharge
In your case, the presence of a small growth (referred to as a "肉芽" or polyp) and the recent development of a rash suggest that there may be an underlying skin condition or irritation that requires attention.

Treatment Options
1. Good Hygiene Practices: Maintain proper hygiene by gently cleaning the area with mild soap and water. Avoid using scented products that can irritate the skin.

2. Topical Treatments: Over-the-counter hydrocortisone creams can help reduce inflammation and itching. However, it’s essential to use these sparingly and not for prolonged periods without medical advice.

3. Antifungal Creams: If a fungal infection is suspected, antifungal creams may be beneficial.

4. Dietary Changes: Identifying and avoiding foods that trigger symptoms can be helpful. Keeping a food diary may assist in pinpointing irritants.

5. Avoid Scratching: Scratching can worsen the condition and lead to secondary infections. Keeping the area dry and using cold compresses can help alleviate itching.

6. Consult a Healthcare Provider: Given your symptoms, especially the presence of a rash and the small growth, it is advisable to consult a healthcare professional. They can perform a thorough examination and may recommend treatments such as prescription medications or further diagnostic tests.

Conclusion
While using a topical ointment that was prescribed for another condition may provide temporary relief, it is crucial to address the underlying cause of your anal itching. Given the complexity of your symptoms, including the history of constipation and the presence of a growth, a professional evaluation is recommended. This will ensure that you receive an accurate diagnosis and appropriate treatment, helping you to manage your symptoms effectively and improve your quality of life. Don't hesitate to seek medical advice; healthcare providers are accustomed to discussing sensitive issues like this and can provide the support you need.
