Experiencing a sensation of incomplete bladder emptying during urination?
Hello Doctor, I had a health check-up at the end of July and found some issues with my prostate.
The report shows: > Prostate-Specific Antigen (PSA) 1.40 > Mild Prostatic Hyperplasia (Size: 2.7*4.37*3.74cm => 23.079mL) > Irregular surface at the right lobe, Nature -> Elastic.
The doctor explained that the texture felt soft upon examination, so there is currently no need for a biopsy unless the texture changes to hard in the future.
Over the past six months, I have noticed a feeling of incomplete bladder emptying.
Last week, I visited the urology department to discuss and follow up on the report.
The doctor prescribed Chuangliqing D 0.2mg, one tablet daily for seven days.
After one week of taking the medication, the issue of incomplete bladder emptying has indeed improved, and I have not experienced any adverse effects from the medication.
During my follow-up visit yesterday, the doctor prescribed another month of Chuangliqing D 0.2mg.
I would like to ask the doctor: 1.
If the issue of incomplete bladder emptying improves, can I stop the medication? Or will I need to take it for life? 2.
Does long-term use of Chuangliqing D have a delaying effect on prostate enlargement? Are there any health impacts from long-term use? 3.
Is the irregular surface of the right lobe related to prostatic hyperplasia, and what is the clinical probability of malignancy in the irregular area? 4.
How often should I follow up on the irregular surface of the right lobe? Is a digital rectal exam sufficient, or should it be accompanied by a transrectal ultrasound? Thank you for your response.

Hello: 1.
It is recommended to take the medication for a period of time, make lifestyle adjustments, and then try to discontinue it; otherwise, the condition may return quickly.
2.
Relaxation is important and should not be minimized.
3.
It is difficult to predict.
4.
Blood tests should be done once a year, digital rectal exams every six months, and ultrasounds annually, of course, based on the outpatient physician's recommendations.
The family history of prostate cancer in your father or grandfather also affects the follow-up intervals.

More Info
Understanding prostate health is crucial, especially when faced with symptoms and diagnostic findings that may indicate underlying issues. Based on your report, you have mild prostate enlargement (benign prostatic hyperplasia, BPH) and a slightly irregular surface on the right lobe of the prostate. Your PSA level is within a normal range, which is reassuring, but it’s essential to monitor your condition closely.
1. Medication Management
You mentioned that you have been prescribed Tamsulosin (Chuang Li Jing D), which is commonly used to alleviate urinary symptoms associated with BPH. If you notice an improvement in your symptoms, it’s understandable to wonder if you can stop the medication. In general, if your symptoms improve significantly, you may discuss with your doctor the possibility of tapering off the medication. However, many patients with BPH find that they need to continue medication long-term to manage symptoms effectively. Stopping the medication may lead to a return of symptoms, so any decision should be made in consultation with your healthcare provider.
2. Long-term Effects of Tamsulosin
Long-term use of Tamsulosin can help manage symptoms of BPH and may slow the progression of prostate enlargement. However, like any medication, it can have side effects. Common side effects include dizziness, fatigue, and potential impacts on sexual function. It’s essential to have regular follow-ups with your doctor to monitor for any adverse effects and to assess the ongoing need for the medication.
3. Irregular Surface and Cancer Risk
The irregular surface of the prostate can be concerning, as it may indicate changes that warrant further investigation. While benign conditions like BPH can cause irregularities, any abnormal findings should be monitored closely. The risk of cancer associated with an irregular surface can vary widely based on individual factors, including family history, age, and other risk factors. Generally, if the prostate feels soft and there are no alarming symptoms (like significant changes in PSA levels or new symptoms), the immediate risk may be lower. However, it’s crucial to have ongoing discussions with your urologist about the implications of these findings.
4. Follow-up and Monitoring
Regarding follow-up, the frequency of monitoring will depend on your specific situation. Typically, if there are no significant changes in your condition, a follow-up every 6 to 12 months may be appropriate. This could include a digital rectal exam (DRE) and possibly a PSA test. If there are concerns about the irregular surface or if your symptoms worsen, more frequent monitoring or additional imaging studies, such as a transrectal ultrasound, may be warranted.
Conclusion
In summary, managing prostate health involves a combination of medication, regular monitoring, and open communication with your healthcare provider. It’s essential to address any concerns about medication duration, potential side effects, and the implications of your prostate findings. Always consult your urologist for personalized advice and to ensure that you are receiving the most appropriate care for your situation. Regular follow-ups and being proactive about your health can help you manage symptoms effectively and maintain your quality of life.
