Concerns About Retained Bone Screws After Orthodontic Treatment - Dentistry

The bone screws were not removed after the correction?

Hello, Doctor.
I have been undergoing orthodontic treatment for almost five years, and I finally had my braces removed over three months ago.
Since my retainers have become loose, I returned for a follow-up appointment to have new impressions taken.
I also wanted to schedule an appointment to remove the bone screws on the left and right sides of my upper arch.
However, the doctor at the clinic responded that since my teeth are not yet stable and there is still a chance for the upper teeth to move up to reduce gum exposure, the screws should not be removed yet.

I would like to ask:
1.
Will leaving the bone screws in for too long after completing orthodontic treatment harm my teeth?
2.
Without the force from braces and rubber bands, is there still a possibility for the teeth to move upward with just the bone screws in place?

Suchin, 20~29 year old female. Ask Date: 2023/09/12

Dr. Zeng Guanghui reply Dentistry

Hello Ms.
Suchin: 1.
If the orthodontic results have not yet stabilized, temporarily leaving the bone screws in place will not harm the teeth, as long as proper oral hygiene is maintained.
2.
It is recommended to consult with an orthodontist.

After undergoing orthodontic treatment for approximately five years, it is understandable to have concerns regarding the retention of bone screws (also known as mini-implants or temporary anchorage devices) after the removal of braces. These screws are often used to provide additional anchorage during treatment, allowing for more effective tooth movement. Here are some insights regarding your concerns:
1. Will leaving the bone screws in for too long harm my teeth?
Generally, leaving bone screws in place after orthodontic treatment does not directly harm the teeth. However, there are a few considerations to keep in mind. First, the primary purpose of these screws is to provide stability during the movement of teeth. Once the orthodontic treatment is completed and the teeth have reached their desired positions, the screws may no longer be necessary. Prolonged retention of these screws can lead to potential complications such as infection, irritation of the surrounding tissues, or discomfort. Additionally, if the screws are left in place for an extended period, there is a possibility that they could interfere with future dental procedures or hygiene practices. Therefore, it is advisable to follow your orthodontist's recommendations regarding the timing of their removal.

2. Can teeth still move upward with just the bone screws in place, without braces or rubber bands?
The potential for tooth movement after the removal of braces largely depends on the stability of the teeth and the surrounding periodontal tissues. While bone screws can provide anchorage, they do not exert active forces on the teeth once the braces and rubber bands are removed. If the teeth are stable and have settled into their new positions, the likelihood of significant movement occurring solely due to the presence of bone screws is minimal. However, if the teeth have not fully stabilized, there may still be a chance for minor adjustments to occur. It is essential to maintain regular follow-up appointments with your orthodontist to monitor the stability of your teeth and to determine the appropriate timing for the removal of the screws.

In summary, while leaving bone screws in place after orthodontic treatment is generally safe, it is crucial to have them evaluated by your orthodontist to ensure that they are still necessary and to prevent any potential complications. Additionally, the movement of teeth after the removal of braces is primarily influenced by the stability of the teeth and the surrounding tissues, rather than the presence of bone screws alone. Regular follow-ups and communication with your orthodontist will help address any concerns and ensure the best outcomes for your dental health.
