Anal canal swelling, pain, itching, and the presence of white granules?
Hello doctor, I have recently experienced swelling in the anal canal and difficulty with bowel movements.
I have been taking anti-inflammatory medication and applying hemorrhoid cream, and it has gradually relaxed.
Usually, I experience redness, swelling, pain, and itching, but recently, as it has loosened, I noticed white particles inside that cause itching when pressed.
I would like to ask if this is due to HPV or inflammation.
I have attached a picture for your reference.
Thank you!
R, 20~29 year old female. Ask Date: 2023/09/15
Dr. Zhang Shuhao reply Surgery
The swollen and prolapsed tissue in the image is indicative of hemorrhoidal tissue, but the white granules are unclear and may suggest inflammation with pus accumulation, or other infectious skin lesions.
It is recommended to visit a colorectal clinic for examination.
If there is an HPV infection, early treatment is necessary.

More Info
Understanding anal canal swelling, particularly when accompanied by pain, itching, and the presence of white bumps, can be quite concerning. The symptoms you describe may be indicative of several conditions, and it’s essential to consider both infectious and non-infectious causes.
Possible Causes
1. Hemorrhoids: Swelling in the anal canal is often associated with hemorrhoids, which are swollen veins in the lower rectum and anus. They can cause pain, itching, and difficulty during bowel movements. The presence of white bumps could be thrombosed hemorrhoids, which occur when a blood clot forms in an external hemorrhoid.
2. Anal Fissures: These are small tears in the lining of the anus, often resulting from passing hard stools. They can cause significant pain and may lead to swelling and itching. While fissures typically do not present as white bumps, they can coexist with other conditions.
3. Skin Tags: After inflammation or irritation, skin tags can develop around the anal area. These are benign and may appear as small, soft bumps. They are usually painless unless irritated.
4. Infections: Conditions such as anal warts caused by the human papillomavirus (HPV) can present as white bumps. HPV can lead to the development of genital warts, which may appear in the anal region. Other infections, such as fungal infections or sexually transmitted infections, can also cause similar symptoms.
5. Inflammatory Conditions: Conditions like dermatitis or psoriasis can cause itching and irritation in the anal area, leading to swelling and the formation of bumps.
6. Abscesses: An anal abscess is a collection of pus that can occur due to infection. It can cause significant swelling, pain, and discomfort, and may require medical intervention.
Diagnostic Approach
To determine the exact cause of your symptoms, a thorough examination by a healthcare professional is necessary. Here are some steps that may be taken:
- Physical Examination: A doctor will perform a physical examination of the anal area to assess for hemorrhoids, fissures, or other abnormalities.
- Medical History: Discussing your symptoms, duration, and any associated factors (such as recent bowel habits, pain levels, and any other health conditions) will help in forming a diagnosis.
- Diagnostic Tests: In some cases, further tests such as a biopsy of the bumps, anal manometry, or imaging studies may be required to rule out other conditions.
Treatment Options
1. Medications: Over-the-counter topical treatments for hemorrhoids, anti-inflammatory medications, or prescription medications for infections may be recommended based on the diagnosis.
2. Sitz Baths: Warm sitz baths can help relieve discomfort and promote healing in the anal area.
3. Dietary Changes: Increasing fiber intake and hydration can help soften stools and reduce straining during bowel movements, which can alleviate symptoms related to hemorrhoids and fissures.
4. Surgical Options: If conservative treatments fail, surgical options may be considered for hemorrhoids or other conditions.
Conclusion
While your symptoms may be related to a benign condition such as hemorrhoids or skin tags, the presence of white bumps raises the possibility of an infectious process, such as HPV. It is crucial to consult a healthcare provider for an accurate diagnosis and appropriate treatment. Early intervention can prevent complications and improve your quality of life. Please ensure you follow up with a medical professional who can provide personalized care based on your specific situation.

The photo does show some redness in the skin, but without a physical examination, it's difficult to determine the exact issue.
