Intermittent chest tightness and the sensation of sounds in the lungs when lying down?
Hello doctor, I was diagnosed with COVID-19 three months ago.
After recovering, I occasionally experience shortness of breath and chest tightness, but I do not have a cough.
I visited a family physician and had an electrocardiogram done.
The doctor noted that my left bronchus sounds narrower upon auscultation, but the ECG showed no significant issues.
I was prescribed medication for anxiety and sleep.
Over the past month, the frequency of chest tightness has decreased, but when lying down, I can hear some bubbling sounds in my lungs, although they are not very pronounced.
I would like to ask if this is simply a case of autonomic nervous system dysregulation or if there might be other issues with my lungs or bronchi?

Hello, thank you for your message.
We recommend that you visit the pulmonology outpatient clinic for relevant examinations such as a chest X-ray and pulmonary function tests.
This will help clarify whether the issue is physiological or related to autonomic nervous system conditions such as anxiety.
Thank you for your inquiry.

Based on your description, it seems that you are experiencing intermittent chest tightness and abnormal lung sounds when lying down, which have persisted since your recovery from COVID-19 three months ago. This situation can be concerning, and it’s essential to consider various factors that could contribute to your symptoms.
Firstly, post-COVID-19 syndrome, often referred to as "long COVID," can manifest in a variety of respiratory symptoms, including shortness of breath, chest tightness, and abnormal lung sounds. These symptoms can occur even after the acute phase of the infection has resolved. In some cases, individuals may experience lingering inflammation in the airways or lung tissue, which can lead to sensations of tightness or discomfort in the chest.
The fact that your healthcare provider noted a narrowing of the left bronchus during auscultation suggests that there may be some degree of airway obstruction or inflammation present. This could be due to bronchial hyperreactivity, which is not uncommon after respiratory infections, including COVID-19. It’s also possible that anxiety related to your previous illness could be contributing to your perception of these symptoms, especially if you have been prescribed anxiolytics.
When you lie down, gravity can affect lung function, and some individuals may experience increased difficulty breathing or abnormal lung sounds due to changes in lung mechanics. This can be exacerbated by conditions such as gastroesophageal reflux disease (GERD), which can cause acid to irritate the airways, leading to wheezing or a sensation of tightness in the chest.
To address your concerns, here are some recommendations:
1. Follow-Up with a Specialist: Given your symptoms and the previous findings, it may be beneficial to consult a pulmonologist. They can perform further evaluations, such as pulmonary function tests, imaging studies (like a chest X-ray or CT scan), and possibly a bronchoscopy if indicated. This will help determine if there are any underlying lung conditions that need to be addressed.
2. Monitor Symptoms: Keep a diary of your symptoms, noting when they occur, their severity, and any potential triggers (such as lying down, physical activity, or emotional stress). This information can be valuable for your healthcare provider in diagnosing your condition.
3. Breathing Exercises: Engaging in breathing exercises or pulmonary rehabilitation can help improve lung function and reduce feelings of breathlessness. Techniques such as diaphragmatic breathing or pursed-lip breathing can be beneficial.
4. Manage Anxiety: If anxiety is a contributing factor to your symptoms, consider discussing this with your healthcare provider. Cognitive-behavioral therapy (CBT) or other forms of counseling can be effective in managing anxiety and its physical manifestations.
5. Lifestyle Modifications: Ensure that you maintain a healthy lifestyle, including regular physical activity, a balanced diet, and adequate hydration. Avoiding smoking and exposure to pollutants can also help improve lung health.
6. Consider GERD: If you suspect that acid reflux may be contributing to your symptoms, discuss this with your doctor. Lifestyle changes, dietary modifications, or medications may be necessary to manage GERD effectively.
In summary, while your symptoms could be related to post-COVID-19 effects, anxiety, or other respiratory issues, a thorough evaluation by a specialist is essential to rule out any significant underlying conditions. It’s important to take your symptoms seriously and seek appropriate medical care to ensure your respiratory health is adequately managed.
