Issues After Hemorrhoid Banding for Internal Hemorrhoids
Hello Doctor: I underwent a hemorrhoid ligation procedure on September 21 this year.
I felt a bit uncomfortable at the time, but aside from experiencing significant pain during bowel movements on one particular day, I didn't have any other issues for a few days.
Later, I noticed that the rubber band had passed during another bowel movement, and there was no bleeding, so I assumed the hemorrhoid had fallen off.
However, for the past few days (starting around September 30), I have been experiencing a persistent tight discomfort around the anal area, similar to muscle tension, which is particularly noticeable when sitting.
On the night of September 30, I even had to take pain medication to sleep.
I would like to know if this discomfort is caused by the ligation.
Would using a hemorrhoid cushion improve the situation? Or should I schedule an earlier follow-up appointment? (My doctor initially said to return in three weeks).
Thank you.

After the ligation of internal hemorrhoids, scar tissue will form in the local area, securing the internal hemorrhoids at a higher position in the anal canal.
As a result, there may be a sensation of tightness in the anus, but it usually does not cause pain.
It is advisable to take warm sitz baths and use anti-inflammatory medications.
If symptoms persist, an earlier follow-up appointment is necessary to determine if additional treatment is required.

After undergoing a hemorrhoid banding procedure, it is not uncommon to experience some discomfort as part of the healing process. The symptoms you are describing, such as a tight or uncomfortable sensation around the anal area, especially when sitting, can be attributed to several factors related to the surgery and the healing process.
Firstly, the sensation of tightness or discomfort can be due to muscle tension in the anal sphincter. After the procedure, the body may react to the trauma of surgery by tightening the muscles in that area, which can lead to a feeling of discomfort. This is often exacerbated by sitting for prolonged periods, as the pressure on the area can increase discomfort.
The fact that you experienced significant pain during bowel movements shortly after the procedure is also a common occurrence. Hemorrhoid banding can cause temporary irritation and inflammation in the anal region, which can make bowel movements painful. The rubber band used in the procedure cuts off blood supply to the hemorrhoid, causing it to shrink and eventually fall off, which can lead to discomfort during the healing phase.
Regarding the rubber band you noticed passing during a bowel movement, this is a normal part of the process. It indicates that the hemorrhoid has likely detached as intended. However, the discomfort you are feeling could be a result of residual inflammation or irritation in the area where the hemorrhoid was located.
As for your question about using a hemorrhoid cushion, many patients find that using a specialized cushion can help alleviate pressure on the anal area when sitting, which may reduce discomfort. These cushions are designed to relieve pressure and provide comfort, making it easier to sit for longer periods without exacerbating pain.
It is also important to maintain a soft stool to avoid straining during bowel movements, as straining can lead to increased pain and discomfort. Staying hydrated, consuming a high-fiber diet, and possibly using stool softeners can help in this regard.
In terms of follow-up care, while your doctor advised a three-week follow-up, it is crucial to listen to your body. If the discomfort persists or worsens, or if you experience any new symptoms such as bleeding, increased pain, or signs of infection (such as fever or unusual discharge), it would be wise to contact your healthcare provider sooner than planned. They can assess your healing progress and determine if any additional interventions are necessary.
In summary, post-banding discomfort is a common experience and can be managed with proper care and attention. Using a hemorrhoid cushion may provide relief, and maintaining a soft stool is essential. However, do not hesitate to reach out to your healthcare provider if you have concerns about your recovery or if your symptoms do not improve. Your comfort and health are paramount, and your doctor can provide the best guidance based on your specific situation.
