Irregular Periods After Stopping Diane: A Patient's Concern - Obstetrics and Gynecology

After taking Diane, my period came for 2 weeks?

Hello Doctor, I have hereditary male pattern baldness and polycystic ovary syndrome (PCOS), which caused irregular menstruation.
I took Diane (Diane-35) to regulate my cycle, but I stopped the medication for a month during the summer.
I returned for a follow-up in September and resumed taking Diane.
After finishing the course, my period started on September 20, but as of now (October 7), I have been experiencing intermittent bleeding.
Sometimes it is a normal red color, and other times it is brown, but the amount is light enough to only require a panty liner.
Could this be related to my stopping the medication during the summer? I have searched online but couldn't find any cases that match my situation.
Thank you for your response, Doctor.

Hello, when reusing contraceptive pills and experiencing breakthrough bleeding, there is no need to stop taking the medication; continue to take it as scheduled.
Once the body adjusts to the contraceptive pills, the abnormal bleeding should cease.
However, if irregular bleeding persists for more than three months or if there is heavy bleeding, it is advisable to consult a physician.
Thank you! Wishing you peace and health.

When it comes to understanding irregular periods after stopping Diane-35, it is essential to consider several factors that can influence menstrual cycles, especially in the context of hormonal treatments. Diane-35 is a combination oral contraceptive that contains ethinyl estradiol and cyproterone acetate, which is often prescribed for conditions like polycystic ovary syndrome (PCOS) and androgen-related disorders, such as hereditary hair loss.
Firstly, it is important to recognize that hormonal contraceptives like Diane-35 work by regulating the hormonal balance in the body. When you stop taking these medications, your body may take some time to readjust to its natural hormonal rhythms. This adjustment period can lead to irregularities in your menstrual cycle, including spotting or breakthrough bleeding, which seems to be what you are experiencing.
The fact that you had a regular period on September 20th after resuming Diane-35, followed by intermittent bleeding until October 7th, could be attributed to a few factors:
1. Hormonal Fluctuations: After stopping Diane-35, your body may still be in the process of recalibrating its hormonal levels. This can lead to irregular bleeding patterns, which might manifest as light spotting or changes in the color and consistency of menstrual blood.

2. Withdrawal Bleeding: When you stop taking hormonal contraceptives, the body may experience withdrawal bleeding, which can sometimes be irregular. The presence of both bright red and brownish blood can indicate that your body is shedding the uterine lining at different rates, which is not uncommon after discontinuing hormonal treatment.

3. Underlying Conditions: Since you mentioned having polycystic ovary syndrome (PCOS), it is crucial to consider that this condition itself can lead to irregular menstrual cycles. Even while on medication, PCOS can cause fluctuations in your cycle, and stopping the medication may exacerbate these irregularities.

4. Duration of Use: The length of time you were on Diane-35 before stopping can also play a role. If you were on it for an extended period, your body might need more time to adjust after discontinuation.

5. Stress and Lifestyle Factors: External factors such as stress, changes in weight, diet, or exercise can also impact menstrual regularity. If there have been any significant changes in your lifestyle, these could contribute to the irregular bleeding you are experiencing.

Given these considerations, it is advisable to monitor your symptoms closely. If the irregular bleeding continues or worsens, or if you experience any other concerning symptoms (such as severe pain or heavy bleeding), it would be prudent to consult with your healthcare provider. They may recommend further evaluation to rule out any underlying issues or to discuss alternative treatment options for managing your PCOS and menstrual irregularities.

In summary, the irregular bleeding you are experiencing after stopping Diane-35 is likely related to hormonal adjustments and the underlying condition of PCOS. It is not uncommon for menstrual cycles to take some time to normalize after discontinuing hormonal contraceptives. Keeping track of your cycle and any associated symptoms will be helpful for your healthcare provider in determining the best course of action moving forward.
