Rubber band ligation for internal hemorrhoids?

Hello, I underwent rubber band ligation for internal hemorrhoids on October 12, 2023.
On October 13, I noticed bright red blood during bowel movements, and on October 14, there was both dark red and bright red blood.
I would like to know if this is normal, as seeing dark red blood makes me very anxious.

Allen, 20~29 year old female. Ask Date: 2023/10/14

Dr. Zhang Shuhao reply Surgery

After hemorrhoid ligation, it is common to experience a small amount of bleeding for a few days, which is considered normal.
You can return for your scheduled follow-up appointment.
However, if you experience heavy bleeding, passage of blood clots, feelings of dizziness or palpitations, or unusual anal pain, you should seek emergency care immediately.

After undergoing rubber band ligation for internal hemorrhoids, it is not uncommon to experience some bleeding, especially in the days immediately following the procedure. The presence of bright red blood during bowel movements can be a normal occurrence as the ligated hemorrhoidal tissue begins to necrose and slough off. This process can lead to bleeding, which may vary in color from bright red to darker shades, depending on the source of the bleeding and how long the blood has been in the gastrointestinal tract.

In your case, experiencing bright red blood on the first day after the procedure (10/13) is generally expected, as this can indicate that the ligation has successfully cut off blood supply to the hemorrhoid, leading to its eventual detachment. The presence of dark red blood on the following day (10/14) may also be concerning, but it can be attributed to a few factors. Dark red blood typically suggests that the blood has been in the digestive tract longer, which can happen if there is bleeding from a deeper source or if the blood has mixed with stool.

While some bleeding is normal, it is crucial to monitor the amount and duration of the bleeding. If the bleeding is significant (for example, if you are soaking through pads or experiencing large clots), or if it persists beyond a few days, it is essential to contact your healthcare provider. They may want to evaluate you to ensure that there are no complications, such as infection or excessive tissue necrosis.

Additionally, if you are experiencing any other symptoms, such as severe pain, fever, or changes in bowel habits, these could indicate complications that require immediate medical attention. It is also important to maintain a soft stool consistency to avoid straining during bowel movements, as this can exacerbate bleeding.

Regarding your concern about the dark red blood causing anxiety, it is understandable to feel worried when observing changes in your body post-surgery. However, it is essential to remember that some bleeding can be part of the healing process. If you are ever in doubt, it is always best to err on the side of caution and consult with your doctor. They can provide reassurance and determine if any further intervention is necessary.

In summary, while some bleeding after rubber band ligation for internal hemorrhoids is normal, it is vital to monitor the situation closely. If the bleeding is excessive, persistent, or accompanied by other concerning symptoms, seek medical advice promptly. Your healthcare provider can offer the best guidance based on your specific situation and help ensure a smooth recovery.
