Post-anal fistula surgery
Hello, Doctor.
I underwent anal fistula surgery in June, and it has been four months since then.
I have been monitoring the healing progress of the wound, but currently, there is still a small opening visible when my anus is relaxed, although it is not visible when I tighten it.
I have noticed some granulation tissue forming inside, but the outer area has not yet healed, and there is no discharge from the wound.
I would like to ask if this indicates a slower healing process or if there could be other reasons.
Thank you for your assistance, Doctor!

If the surgery was performed four months ago, the wound should be fully healed by now.
The small hole you described may just be a remnant of the surgical scar.
However, if you still see an unhealed wound, it is best to consult the surgeon again to rule out the possibility of recurrence.

After undergoing anal fistula surgery, it is common to have concerns about the healing process and what to expect during recovery. Based on your description, it has been four months since your surgery, and you are observing a small opening in the anal area that is visible when relaxed but not when tightened. Additionally, you mentioned that there is some granulation tissue forming inside, but the external wound has not fully healed, and there is no discharge.
First and foremost, it is important to understand that the healing process after anal fistula surgery can vary significantly from person to person. Several factors can influence the rate of healing, including the type of fistula, the surgical technique used, individual health conditions, and adherence to post-operative care instructions.
In general, the healing of anal fistula wounds can take several weeks to months. It is not uncommon for the external part of the wound to take longer to heal than the internal part. The presence of granulation tissue is a positive sign, indicating that the body is attempting to heal the wound. However, the fact that there is still a small opening may suggest that the external layer of the tissue is still in the process of closing.
The absence of discharge is a good sign, as it indicates that there is no active infection. However, the small opening you are observing could be a result of several factors:
1. Delayed Healing: Some individuals may experience slower healing due to factors such as poor blood circulation, underlying health conditions (like diabetes), or nutritional deficiencies.
2. Tension on the Wound: Activities that put strain on the anal area, such as heavy lifting or straining during bowel movements, can impede healing. It is crucial to avoid activities that may stress the surgical site.
3. Scar Tissue Formation: Sometimes, scar tissue can form in a way that affects the healing process. This can lead to a situation where the outer layer appears to heal, but the inner layers are still in the process of closing.
4. Surgical Technique: The type of surgical technique used can also impact healing. Some techniques may leave more tissue to heal than others, which can prolong the recovery period.
In terms of what to expect moving forward, it is essential to maintain regular follow-up appointments with your surgeon or healthcare provider. They can assess the healing process and determine if any additional interventions are necessary. If the wound does not show signs of improvement or if you experience increased pain, swelling, or any discharge, it is crucial to seek medical advice promptly.
As for your question about whether the slow healing is normal, it can be within the range of normal healing, but it is always best to consult with your healthcare provider for a personalized assessment. They may recommend specific wound care practices or treatments to promote healing.
In conclusion, while some degree of delayed healing can be expected after anal fistula surgery, ongoing monitoring and communication with your healthcare provider are key to ensuring a successful recovery. Patience is essential, as healing can take time, but with proper care and attention, you should see gradual improvement.
