Urological Concerns: Sensations, Ejaculation, and Urinary Issues - Urology

Fluid discharge

Personal situation: I have had little contact with the opposite sex, have been single for a long time, and have almost no experience with intimate interactions.
I would like to ask the doctor the following questions:
1.
Sometimes when I think of more erotic images, I experience a momentary sensation in my penis or glans that feels like an electric shock (or a current passing through?).
However, there is no erection or discharge of fluid.
Is this related to the nervous system, or what could be causing this? Do I need to seek medical examination?
2.
During ejaculation, I notice that the fluid expelled is mostly prostatic fluid, which is quite watery, and the volume is not much with a short distance.
Afterward, the contractions cause thick, viscous semen to flow out in segments from the urethral opening.
Additionally, I seem to have a phenomenon of "post-urination dribbling." Is this due to insufficient function of the penile urethral muscles, or what could be causing this? Should I seek medical examination?

Niming, 30~39 year old female. Ask Date: 2023/10/26

Hello: 1) Descriptions related to sensations are difficult to address, I apologize.
2) Everyone is different; if there is a functional deficiency, it is recommended to seek medical attention at a large hospital.
Wishing you good health.

Understanding urological concerns can be complex, especially when it involves sensations, ejaculation, and urinary issues. Based on your questions, let's break down the concerns and provide some insights.

1. Sensation of Electric Shock in the Penis
The sensation you describe—feeling like an electric shock in the penis or glans when thinking about erotic images—may be related to the nervous system. This phenomenon can occur due to heightened arousal or stimulation of the sensory nerves in the genital area. The penis is richly supplied with nerve endings, and thoughts or fantasies can trigger a response even without physical stimulation.
This sensation is generally not a cause for concern and does not necessarily indicate a medical issue. However, if you experience persistent discomfort, pain, or other unusual symptoms, it would be wise to consult a healthcare professional. They can perform a thorough evaluation to rule out any underlying neurological or urological conditions.

2. Ejaculation and Semen Characteristics
Regarding your observation of the ejaculate being predominantly prostate fluid, with a small volume and short distance, this can be influenced by several factors. The prostate gland produces a significant portion of the fluid that makes up semen, and variations in the consistency and volume can occur due to hydration levels, frequency of ejaculation, and overall health.

The "dribbling" or "post-void dribbling" you mentioned after urination could be attributed to several factors, including:
- Weak Pelvic Floor Muscles: The muscles that control urination may not be functioning optimally, leading to residual urine in the urethra that dribbles out after you finish urinating.

- Prostate Health: Conditions such as prostatitis or benign prostatic hyperplasia (BPH) can affect urinary function and ejaculation.

- Nerve Function: As with the first concern, the nervous system plays a crucial role in both urination and ejaculation. Any disruption in nerve signaling can lead to issues.

When to Seek Medical Attention
While the sensations and characteristics you've described may not be alarming, it's essential to monitor your symptoms. If you notice any of the following, it would be prudent to seek medical advice:
- Persistent pain or discomfort in the genital area.

- Significant changes in urination patterns (e.g., increased frequency, urgency, or difficulty).

- Blood in urine or semen.

- Any other unusual symptoms that cause concern.

Conclusion
In summary, the sensations you experience may be linked to normal physiological responses and the functioning of the nervous system. The characteristics of your ejaculate and the post-void dribbling could be influenced by various factors, including muscle strength and prostate health. If these issues persist or worsen, consulting a urologist would be beneficial. They can provide a comprehensive evaluation, including physical examinations and possibly imaging or laboratory tests, to ensure your urological health is in good standing. Remember, it's always better to address concerns early rather than waiting for them to escalate.
