Abdominal Pain and Stool Changes: When to Worry - Gastroenterology and Hepatology

Bowel issues

Dear Deputy Director, I would like to ask a question.
Sometimes when I have a bowel movement, I suddenly experience cramping abdominal pain (similar to diarrhea).
The stool that comes out is not watery; it starts as a long, formed shape and the latter part is somewhat loose and mushy.
I would like to know if this is considered a normal diarrhea situation.
After having a bowel movement, I still feel uncomfortable in my abdomen (as if I still need to go), and my anus feels sore.
Could this type of diarrhea be a warning sign of cancer? I usually eat a lot of vegetables and fruits! I tend to wait until I really need to go before I have a bowel movement, so I often pass a large amount.
Thank you for your response, Deputy Director.

It is just stronger bowel motility and is not related to cancer, unless there is blood in the stool or unexplained weight loss.

When it comes to abdominal pain and changes in stool consistency, it's essential to understand the potential causes and when to seek medical attention. Based on your description, it seems you are experiencing a combination of abdominal cramping and changes in your bowel movements, which can be concerning but may not necessarily indicate a severe underlying condition.

Firstly, the nature of your bowel movements—having a formed stool followed by a looser consistency—can be indicative of several gastrointestinal issues. This pattern may suggest a condition known as "irritable bowel syndrome" (IBS), which is characterized by abdominal pain and altered bowel habits, including diarrhea and constipation. IBS is often triggered by dietary factors, stress, and changes in gut motility. Given that you consume a lot of fruits and vegetables, it's possible that the high fiber intake is contributing to your symptoms, especially if your body is not used to such a diet or if you are not drinking enough fluids.

The sensation of wanting to have a bowel movement after you have already gone, along with discomfort in the abdomen, can also be associated with conditions like rectal irritation or inflammation, which may occur due to factors such as hemorrhoids, anal fissures, or even infections. The "sour" feeling in the anus you mentioned could be a result of irritation from frequent bowel movements or straining.

While your symptoms may not immediately suggest cancer, it is crucial to be vigilant. Changes in bowel habits, especially if they are persistent or accompanied by other symptoms such as weight loss, blood in the stool, or severe abdominal pain, warrant further investigation. In your case, the fact that you experience significant abdominal cramping during bowel movements and the sensation of incomplete evacuation could be a sign that you should consult a healthcare provider for a thorough evaluation.

In terms of cancer concerns, while colorectal cancer can present with changes in bowel habits, abdominal pain, and rectal discomfort, it is more commonly associated with other symptoms such as blood in the stool, unexplained weight loss, and changes in appetite. Given your age and health history, it is essential to discuss your symptoms with a healthcare professional who may recommend diagnostic tests such as a colonoscopy, especially if you have a family history of gastrointestinal diseases or cancer.

In summary, while your symptoms may not be immediately alarming, they are certainly worth discussing with a healthcare provider. They can assess your overall health, perform necessary examinations, and determine if further testing is needed. Maintaining a balanced diet, staying hydrated, and managing stress can also help alleviate some of your symptoms. If your symptoms persist or worsen, do not hesitate to seek medical advice.
