Issues with dietary supplements and antihypertensive medications?

Hello, doctor.
I am currently taking 5 mg of Amlodipine daily.
Due to accompanying tinnitus symptoms, I use 2 mg of Diazepam before bed.
Recently, I saw recommendations for a combination of D3 and K2 to reduce calcium ion deposition in the blood.
I found a supplement that combines D3 (5000 IU) and K2 (100 mcg).
I have checked that Amlodipine is a calcium channel blocker.
Can I take this supplement while on Amlodipine during the day? Thank you.

Karel, 40~49 year old female. Ask Date: 2023/11/29

Dr. Li Shengkai reply Cardiology

Upon investigation, Amlodipine (Norvasc) does not have any significant interactions with Vitamin D3.
Therefore, the two medications you mentioned (Amlodipine & Diazepam) and the health supplements (D3 + K2) can be taken together.

More Info

When considering the use of dietary supplements such as Vitamin D3 and K2 alongside prescription medications like Amlodipine (known as 脈優 in Chinese), it is essential to understand both the pharmacological interactions and the physiological effects of these substances.

Amlodipine is a calcium channel blocker commonly prescribed for managing high blood pressure. It works by relaxing the blood vessels, allowing blood to flow more easily, which helps to lower blood pressure. On the other hand, Vitamin D3 (cholecalciferol) and Vitamin K2 (menaquinone) play significant roles in calcium metabolism. Vitamin D3 aids in the absorption of calcium from the diet, while Vitamin K2 is crucial for directing calcium to the bones and teeth and preventing its deposition in soft tissues, such as blood vessels.

From the information provided, there is no significant interaction between Amlodipine and Vitamin D3. This means that taking D3 alongside Amlodipine should not adversely affect the efficacy of the blood pressure medication. Furthermore, Vitamin K2 may actually complement the effects of Vitamin D3 by ensuring that calcium is utilized properly in the body, potentially reducing the risk of vascular calcification, which is a concern for individuals on calcium channel blockers.

Regarding the specific combination of D3 (5000 IU) and K2 (100 mcg), this dosage is generally considered safe for most individuals, including those taking Amlodipine. However, it is always advisable to consult with a healthcare provider before starting any new supplement, especially if you are on multiple medications. This is particularly important for individuals with underlying health conditions or those who are taking other medications that may interact with supplements.

In your case, since you are also taking Diazepam (2 mg) at night for tinnitus, it is worth noting that there are no known interactions between Diazepam and the D3+K2 combination either. Diazepam is a benzodiazepine that works by calming the brain and nerves, and it does not interfere with the metabolism of vitamins D3 and K2.

In summary, you should be able to take the D3+K2 supplement while continuing with Amlodipine and Diazepam without any significant concerns. However, it is crucial to monitor your blood pressure regularly and report any unusual symptoms to your healthcare provider. They can provide personalized advice based on your overall health status and medication regimen.

Additionally, it is essential to maintain a balanced diet rich in nutrients that support cardiovascular health, including omega-3 fatty acids, potassium, magnesium, and antioxidants. Regular physical activity, stress management techniques, and adequate hydration are also vital components of managing high blood pressure effectively.

In conclusion, while the combination of D3 and K2 appears to be safe with your current medications, always prioritize open communication with your healthcare provider regarding any changes to your supplement or medication regimen. They can help ensure that your treatment plan remains effective and safe.
